摘　　要：目的分析不同H-Y分期和是否伴认知功能障碍帕金森病(PD)患者血清同型半胱氨酸(Hcy)和胱抑素C(Cys C)水平变化。方法选取72例PD患者为PD组,30例体检合格的健康人员为对照组,比较2组Hcy、Cys C水平。根据PD组H-Y分期的不同,分为早期组(21例)、中期组(24例)、晚期组(27例)。并再将72例PD患者进行亚组分析,伴有认知功能障碍者31例为Ⅰ组,认知功能正常者41例为Ⅱ组。比较各组血清Hcy、Cys C水平的情况,并分析患者血清Hcy、Cys C水平与性别、年龄、体质量指数(BMI)、病程、H-Y分期的关系。结果 PD患者中晚期及早期血清Hcy、Cys C水平较对照组均显著升高(P<0.05);中、晚期组血清Hcy、Cys C水平较早期组均显著升高(P<0.05);Ⅰ组血清Hcy、Cys C水平较Ⅱ组均显著升高(P<0.05)。PD患者血清Hcy、Cys C水平与性别、年龄、BMI均无显著相关性(P>0.05),而与H-Y分期、病程均密切相关(P<0.05)。结论不同H-Y分期的PD患者血清Hcy、Cys C水平存在明显差异,并且其在伴有认知功能障碍患者中的水平显著升高,提示血清Hcy、Cys C可能参与PD疾病的发生与发展过程。Objective To analyze the changes of serum homocysteine(Hcy) and cystatin C(Cys C) levels in patients with Parkinson’s disease(PD) at different stages of H-Y and cognitive impairment.Methods 72 cases of PD in our hospital were selected as the PD group, and 30 cases of qualified health persons were selected as the control group. According to the different stages of H-Y in PD group, 72 cases were divided into the early stage group(21 cases), medium stage group(24 cases) and advanced stage group(27 cases). Among of 72 cases, 31 patients with cognitive impairment as the group I and 41 patients with normal cognitive function as the groupⅡ. The levels of serum Hcy and Cys C in those groups were compared, and the relationship between serum levels of Hcy, Cys C and gender, age, body mass index(BMI), course of disease and H-Y stage were analyzed.Results The serum Hcy and Cys C levels in the PD group were significantly higher than that of control group(P<0.05). The serum Hcy and Cys C levels in the medium and advanced group were significantly higher than those in early stage group(P<0.05). The serum Hcy and Cys C levels in the group I were significantly higher than those in the group II(P<0.05). The serum levels of Hcy and Cys C in patients with PD were not significantly correlated with gender, age, and BMI(P>0.05), but closely related to H-Y stage and course of disease(P<0.05).Conclusions There are obvious differences in serum Hcy and Cys C levels at different H-Y stage of PD patients, and its levels in patients with cognitive dysfunction are significantly increased which suggest that it may be involved in PD disease. It may be used to predict the progression of the disease degree.
