血清肌钙蛋白T、心肌酶及白介素-6联合检测对慢性心衰的诊断价值

摘　　要：目的观察分析血清肌钙蛋白T(cTnT)、心肌酶及白介素-6(IL-6)联合检测对慢性心衰患者的诊断价值。方法选择2016年7月至2019年6月期间在南京医科大学第三附属医院(附属逸夫医院)进行治疗的400例慢性心衰患者作为研究组,选择同期在该院进行体检的健康人群300例作为对照组,分别对两组进行cTnT、有肌酸激酶同工酶(CK-MB)、肌酸激酶(CK)、乳酸脱氢酶(LDH)、谷草转氨酶(AST)、纽约心脏病协会(NYHA)等指标检测,对检测结果进行分析比较。结果经过检测后发现,研究组患者cTnT、CK-MB、CK、LDH、AST、IL-6等指标明显高于对照组(P<0.05);研究组中,NYHA心功能III级与IV级cTnT、IL-6指标异常检出率比较差异无统计学意义(P>0.05),NYHA心功能III级CK-MB、CK、LDH、AST异常检出率明显低于IV级(P<0.05)。结论慢性心衰患者的cTnT、心肌酶、IL-6等指标明显高于正常人群,随着心衰病情进展,各指标呈明显升高趋势,在临床诊断治疗中具有重要的指导意义。Objective To observe and analyze the diagnostic value of combined detection of serum troponin T(cTnT),myocardial enzymes and interleukin-6(IL-6)in patients with chronic heart failure.Methods 400 cases of patients with chronic heart failure and who treated in The Third Affiliated Hospital of Nanjing Medical University(Sir Run Run Hospital)from July 2016 to June 2019 were selected as the study group,and other 300 cases of healthy people who underwent physical examination in our hospital in the same period were selected as control group.Then,the indexes of cTnT,CK-MB,CK,LDH,AST and IL-6 were detected between the two groups,and the results were analyzed and compared.Results After the detection,the indexes of cTnT,CK-MB,CK,LDH,AST and IL-6 in the study group were significantly higher than those in the control group(P<0.05).In the study group,the abnormal detection rates of cTnT and IL-6 in NYHA grade Ⅲ and Ⅳ were compared(P>0.05).And the abnormal detection rates of CK-MB,CK,LDH and AST in NYHA grade Ⅲ were significantly lower than those in grade IV(P<0.05).Conclusion The indexes of cTnT,myocardial enzymes and IL-6 in patients with chronic heart failure are significantly higher than those in normal people.With the progress of heart failure,the indexes showed a significant upward trend,thus it has important guiding significance in clinical diagnosis and treatment.
