摘 要:糖尿病性心肌病(DCM)为独立于其他主要心血管危险因素的心脏功能障碍,其潜在原因未明,已成为糖尿患者群中心力衰竭的主要原因。其致病因素多样,但主要源于胰岛素抵抗或胰岛素信号传导受损所致的代谢失灵活,以致葡萄糖氧化受损及绝对依赖于脂肪酸供能,对心肌细胞功能产生有害影响。本文综述了糖尿病个体中心脏胰岛素抵抗及胰岛素信号传导受损在DCM发病中的作用进展。Diabetic cardiomyopathy(DCM)is a cardiac dysfunction independent of other major cardiovascular risk factors.Its underlying cause is unknown and has become the main cause of central failure in people with diabetes.The pathogenic factors of DCM are diverse,but it is mainly due to metabolic inflexibility caused by insulin resistance or impaired insulin signaling,which lead to impaired glucose oxidation and absolute reliance on fatty acid energy,and has a deleterious effect on cardiac muscle cell function.This article reviews the role of cardiac insulin resistance and impaired insulin signaling in the pathogenesis of DCM in diabetic individuals.
