摘  要:目的探讨丙氨酸氨基转移酶(ALT)和γ-谷氨酰基转移酶(GGT)水平与新诊断2型糖尿病(T2DM)患病之间的关联,为T2DM的防治提供科学依据。方法于2006年1月和2009年1月采用4阶段分层随机抽样的方法,共选取青岛地区7819名35~74岁成年居民作为调查对象,进行问卷调查、体格检查和实验室检测。采用SPSS 23.0软件进行t检验、χ^2检验,ALT、GGT水平与空腹血糖(FPG)、餐后2 h血糖(2 h PG)的相关性采用Pearson相关分析,新诊断T2DM患病的影响因素采用多因素非条件logistic回归分析。结果男性新诊断T2DM患病率(13.10%)高于女性(10.70%),女性ALT、GGT水平低于男性,差异均有统计学意义(P<0.01)。Pearson相关分析结果显示,ALT、GGT水平均与FPG(r值分别为0.077、0.115)、2 h PG(r值分别为0.078、0.111)呈正相关,均有统计学意义(P<0.01)。多因素logistic回归分析结果显示,调整混杂因素后,总人群ALT异常(OR=2.48,95%CI:1.75~3.52)、GGT异常(OR=2.83,95%CI:2.24~3.59)均与新诊断T2DM高患病风险相关(P<0.01);男性、女性ALT异常(男性OR=2.68,95%CI:1.57~4.59;女性OR=2.32,95%CI:1.46~3.69)、GGT异常(男性OR=2.48,95%CI:1.84~3.35;女性OR=3.35,95%CI:2.27~4.95)均与新诊断T2DM高患病风险相关(P<0.01)。结论ALT、GGT异常与T2DM高患病风险相关,后续还需要长期研究或干预试验进一步验证。Objective To explore the association of alanine aminotransferase(ALT),γ-glutamyltransferase(GGT)levels with the risk of newly diagnosed type 2 diabetes mellitus(T2DM),and to provide the basis for preventing and treating the T2DM.Methods From January of 2006 to January of 2009,a four-stage stratified random sampling method was used to select 7819 adults(35-74 years old)in Qingdao city as the subjects.The investigation was performed by questionnaire,physical examination and laboratory test.The t test andχ^2 test were used to analyze the data,the Pearson correlation was used to analyze the correlation between ALT or GGT levels and fasting plasma glucose(FPG)or 2 h post-load plasma glucose(2 h PG).Multivariate unconditional logistic regression was used to analyze the influencing factors of T2DM.The used software was SPSS 23.0.Results The morbidity of newly diagnosed T2DM in males was 13.10%which was significantly higher than that in females(10.70%),the levels of ALT and GGT in females were significantly lower than those in males,P<0.01.Pearson correlation illustrated that ALT and GGT levels were positively correlated with FPG(ALT:r=0.077,GGT:r=0.115)and 2 h PG(ALT:r=0.078,GGT:r=0.111),P<0.01.Multivariable logistic regression revealed that after adjusting confounding factors,abnormal ALT(OR=2.48,95%CI:1.75-3.52)and abnormal GGT(OR=2.83,95%CI:2.24-3.59)were related to high risk of newly diagnosed T2DM(P<0.01);abnormal ALT was related to high risk of newly diagnosed T2DM in males(OR=2.68,95%CI:1.57-4.59)and in females(OR=2.32,95%CI:1.46-3.69);and abnormal GGT was related to high risk of newly diagnosed T2DM in males(OR=2.48,95%CI:1.84-3.35)and in females(OR=3.35,95%CI:2.27-4.95),P<0.01.Conclusion The abnormal ALT and GGT were related to higher risk of T2DM.In further,longterm studies or intervention trials should be needed.
