摘 要:目的探索佩戴踝足矫形器对脑卒中后足下垂的影响。方法选取2015年4月至2018年12月踝关节主动背屈不能达到90°中立位的脑卒中患者20例作为研究对象。采用随机数字法分为观察组和对照组,每组各10例。2组患者均给予药物治疗和康复训练,观察组在此基础上佩戴踝足矫形器(AFO),每天不少于8h。于治疗前、治疗后分别采集三维步态数据,评估患者足下垂及步行功能改善情况。结果治疗前、后2组患者踝关节活动度比较差异均无统计学意义(P>0.05)。治疗后观察组踝关节活动度较治疗前明显改善,差异有统计学意义(P<0.01)。治疗前、后2组患者步行速度比较差异均无统计学意义(P>0.05)。治疗后2组患者步行速度均较治疗前显著提高,差异均有统计学意义(P<0.01)。观察组步行速度变化率高于对照组,差异有统计学意义(P<0.05)。结论足下垂患者佩戴AFO,能提高步行中踝关节活动度,改善步行功能。Objective To explore the effect of wearing ankle and foot orthosis on foot drop after stroke.Methods Twenty patients with stroke who were unable to achieve 90°neutral active dorsiflexion from April 2015 to December 2018 were selected as the research objects.Random number method was used to divide observation group and control group,with 10 cases in each group.Patients in both groups were given medication and rehabilitation training.On the basis of this,the observation group wore an ankle and foot orthosis(AFO)for no less than 8 hours per day.Three-dimensional gait data was collected before and after treatment to evaluate the improvement of patient’s foot drop and walking function.Results There was no significant difference in ankle joint activity between the two groups before and after treatment(P>0.05).After treatment,the ankle joint activity in the observation group was significantly improved compared with that before treatment,and the difference was statistically significant(P<0.01).There was no significant difference in walking speed between the two groups before and after treatment(P>0.05).After treatment,the walking speed of the two groups was significantly higher than that before treatment,and the differences were statistically significant(P<0.01).The rate of change in walking speed in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Wearing AFO in patients with sagging feet can improve ankle joint mobility and improve walking function during walking.
