多囊卵巢综合征患者内分泌状态与宫腔内人工授精成功率的相关性分析

摘　　要：目的探讨多囊卵巢综合征(PCOS)患者内分泌状态与宫腔内人工授精(IUI)成功率的相关性。方法回顾性分析2017年3月-2018年3月在郑州大学第三附属医院接受IUI助孕的300例PCOS患者的临床资料,共468个周期。按照抗缪勒管激素(AMH)值将所有入选者分为A组(AMH≤16.32 pmol/L,74例)、B组(16.32 pmol/L29.35 pmol/L,73例)。对比分析3组患者的高密度脂蛋白胆固醇(High Density Lipoprotein,HDL-C)、卵泡刺激素(Follicle Stimulating Hormone,FSH)、黄体生成素(Luteinizing Hormone,LH)水平及妊娠率。结果 C组患者HDL-C、FSH、LH水平均显著高于A组、B组患者,差异有统计学意义(P<0.05);A组患者的妊娠率为29.73%,B组患者的妊娠率为16.99%,C组患者的妊娠率为11.11%,3组患者的妊娠率比较,差异无统计学意义(P>0.05);采用Pearson相关性分析发现,AMH值与HDL-C、FSH及LH水平呈正相关(r>0,P<0.05)。结论 AMH患者的HDL-C、FSH、LH水平均明显较高,但其对多囊卵巢综合征患者的人工授精成功率的判断价值有限。Objective To investigate the relationship between endocrine status and the success rate of intrauterine insemination(IUI) in patients with polycystic ovary syndrome(PCOS). Methods The clinical data of 300 patients with PCOS who underwent IUI in the hospital from March, 2017 to May, 2018 were retrospectively analyzed for a total of 468 cycles. According to the anti-Mullerian hormone(AMH) value, all the subjects were divided into group A(AMH≤16.32 pmol/L, 74 cases), group B(16.32 pmol/L29.35 pmol/L, 73 cases). The high-density lipoprotein cholesterol(HDL-C),Follicle Stimulating Hormone(FSH), Luteinizing Hormone(LH) levels and pregnancy rate were compared among the three groups.Results The levels of HDL-C, FSH and LH in group C were significantly higher than those in group A and B. The difference was statistically significant(P<0.05). The pregnancy rate in group A was 29.73%. The pregnancy rate in group B was 16.99%, and the pregnancy rate of group C was 11.11%. There was no significant difference in pregnancy rate among the three groups(P>0.05). Pearson correlation analysis showed that there was a positive correlation between AMH hormone value and HDL-C FSH and LH levels(r>0,P<0.05). Conclusion The levels of HDL-C, FSH and LH in patients with high AMH value are significantly higher, but their value in judging the success rate of artificial insemination in patients with polycystic ovary syndrome is limited.
