摘　　要：目的探讨新型冠状病毒肺炎首次HRCT影像学特征。方法由两名以上影像医师分别阅片,回顾性分析救治21例新型冠状病毒核酸检测阳性患者临床及影像学资料,观察胸部HRCT是否有阳性表现,并记录病变的部位、范围及影像特点。结果胸部HRCT可计数病例共计17例,发现病灶共56个;不可计数3例,表现为两肺弥漫病变。左肺受累6例(6/21),右肺受累3例(3/21),双肺同时累及11例(11/21)。病灶可表现为片状磨玻璃影、磨玻璃小结节、磨玻璃与实变混杂影、实性小结节、小叶间隔增厚及纤维化;局限性胸膜增厚为本病的不典型表现;胸腔积液为少见伴随征象。结论新型冠状病毒肺炎有一定特征性影像表现,对于有流行病学史并有上呼吸道症状患者应早期行HRCT检查。Objective To investigate the imaging characteristics in patients with novel coronavirus pneumonia in the first HRCT examination.Methods The clinical data and imaging data about 21 patients with novel coronavirus pneumonia whose novel coronavirus nucleic acid test was positive were etrospectively analyzed.More than two radiologists reviewed the CT examination results independently to retrospectively analyze whether the chest HRCT was positive,and the location,scope and imaging characteristics of the lesions were recorded.Results Chest HRCT found 56 countable lesions in 17 patients,and 3 patients presented with uncountable diffuse lesions in both lungs.There were 6 cases(6/21)of left lung involvement,3 cases(3/21)of right lung involvement,and 11 cases(11/21)of both lungs involvement.The lesions could be shown as ground-glass opacities,ground glass nodules,ground-glass and consolidation in combination,consolidation nodule,interlobular septal thickening and fibrosis.And the localized pleural thickening was an atypical manifestation of this disease,and pleural effusion was a rare associated sign.Conclusion The novel coronavirus pneumonia has certain characteristic imaging manifestations,it is necessary to perfom chest HRCT as soon as possible for the patients with epidemiological history and upper respiratory symptoms.
