甲状腺结节钙化的超声分类表现及钙化与病理组织学结果的关系

摘　　要：目的探讨甲状腺结节内钙化的超声分类表现及钙化与病理组织学结果的关系。方法根据甲状腺钙化的大小、形态和区域将钙化分为六种超声类型,根据病理组织学检查结果对甲状腺结节的良恶性钙化发生率和超声类型进行分析比较。结果彩超提示甲状腺钙化的敏感性为87.5%,特异性为98.9%;钙化发生率在良性结节中为17.3%,在恶性结节中为51.3%,钙化的发生率在恶性结节中比例较高(P<0.001);微钙化和小钙化在恶性甲状腺结节内发生率比较高(P<0.001),主要发生在甲状腺乳头状癌中。结论甲状腺钙化对于鉴别诊断甲状腺结节良恶性具有重要临床意义,超声检查微钙化及小钙化是甲状腺乳头状癌的特征性表现之一。Objective To investigate the ultrasonographic classification of calcification in thyroid nodules and the relationship between calcification and histopathological results. Methods According to the size, shape and region of thyroid calcification, the calcification was divided into six ultrasound types, and the incidence and ultrasound types of benign and malignant calcification of thyroid nodules were analyzed and compared according to the results of histopathological examination. Results The sensitivity and specificity of thyroid calcification were 87.5% and 98.9% respectively. The incidence of calcification was 17.3% in benign nodules and 51.3% in malignant nodules, and the incidence of calcification was higher in malignant nodules(P<0.001). The incidence of microcalcification and small calcification in malignant thyroid nodules was relatively high(P<0.001), which mainly occured in papillary carcinoma of thyroid(P<0.001). Conclusion Thyroid calcification is an important clinical significance in the differential diagnosis of benign and malignant thyroid nodules. Ultrasound examination of microcalcification and small calcification is one of the characteristic manifestations of papillary carcinoma of thyroid.
