摘　　要：目的探讨经腹联合经阴道超声诊断子宫瘢痕处胎盘植入价值。方法选取深圳市宝安区人民医院于2017年1月-2019年1月间收治的疑似子宫瘢痕处胎盘植入患者100例,所有患者均接受经腹超声和经阴道超声诊断,并经手术病理结果确诊,观察分析单纯经腹部超声与经腹联合经阴道超声诊断准确率、特异度及敏感度。结果 100例疑似子宫瘢痕处胎盘植入患者中61例经手术病理确诊为阳性,39例确诊为阴性;单纯经腹部超声诊断45例阳性,28例阴性;经腹联合经阴道超声诊断59例阳性,38例阴性。经腹联合经阴道超声诊断子宫瘢痕处胎盘植入的诊断准确率、特异度和敏感度均比单纯经腹超声明显更高(P<0.05)。结论临床诊断子宫瘢痕处胎盘植入采用经腹联合经阴道超声能够有效提高诊断效能,值得推广。Objective To probe into the value of transabdominal combined transvaginal ultrasound in diagnosis of placenta implantation in uterine scar. Methods 100 patients with placenta implantation in uterine scar who treated and suspected from January,2017 to January,2019 were selected for analysis. All patients were received transabdominal and transvaginal ultrasound diagnosis,and the diagnosis was confirmed by operation and pathology. Then,the diagnostic accuracy, specificity and sensitivity of transabdominal ultrasound and transabdominal combined transvaginal ultrasound were observed and analyzed. Results 100 patients suspected of placenta implantation in uterine scar, 61 cases were diagnosed as positive by operation and pathology, and 39 were diagnosed as negative. 45 cases were positive and 28 cases were negative by abdominal ultrasonography alone. 59 cases were positive and 38 cases were negative by transabdominal combined transvaginal ultrasound diagnosis. The diagnostic accuracy, specificity and sensitivity of transabdominal combined transvaginal ultrasound in the diagnosis of placenta implantation in uterine scar were significantly higher than those of transabdominal ultrasound alone(P<0.05). Conclusion The transabdominal combined transvaginal ultrasound in clinical diagnosis of placenta implantation in uterine scar can effectively improve the diagnostic efficiency,thus it is worth promotion.
