《吴鞠通医案》湿病组方规律及探讨

摘　　要：目的基于中医传承辅助平台软件,挖掘吴鞠通《温病条辨》[1]《吴鞠通医案》[2]治疗湿病的组方规律,为当代医家临证提供处方思路。方法将《温病条辨》《吴鞠通医案》中治疗湿病的处方录入中医传承辅助系统,对高频药物进行统计,并对其进行关联分析。结果对筛选出的154首治疗湿病的处方进行分析,发现吴鞠通治疗湿病的高频药物为茯苓、厚朴、人参、附子、杏仁、半夏、桂枝等,药性多为辛、苦、甘,归经多属脾、胃、肺。对出现的高频药物进行关联分析,得出关联较大的药对,如半夏-茯苓,茯苓-薏苡仁,厚朴-茯苓,茯苓-杏仁等。结论从中医传承辅助平台的多角度分析,表明吴鞠通治疗湿病多从三焦辨治,处方以宣化肺气、健脾燥湿、利水渗湿为治疗总则,且重视顾护中州脾胃,对临床实践有一定的指导意义。Objective Based on traditional Chinese medicine inheritance system,to analyze the composing principles of the prescriptions for treating damp disease in the Item Differentiation of Warm Febrile Diseases and Wu Jutong’s Collection of Case Histories,and provide guidance for contemporary doctors’clinical use of herbals.Methods Inputting the prescriptions for treating damp disease in the Item Differentiation of Warm Febrile Diseases and Wu Jutong’s Collection of Case Histories into the TCM inheritance system,statistics of herbals used frequently and make associative analysis of the high-frequency herbals was made.Results Based on the analysis of 154 cases of prescriptions,we found that herbals used by Wu Jutong frequently in damp disease treatment are poria cocos,mangnolia officinalis,ginseng,monkshood,apricot kernel,pinellia tuber,cassia twig,etc.Most of these high-frequency herbals are bitter and sweet.Meridian tropism is mostly spleen,stomach and lung.Through associative analysis of the high-frequency herbals,there are several herbal pairs that are closely correlated,such as pinellia tuber-poria cocos,poria cocos-semen coicis,mangnolia officinalis-poria cocos,poria cocos-apricot kernel,etc.Conclusion The analysis showed that Wu Jutong laid stress on triple energizer syndrom differentiation and the composing principles are mainly ventilating the stagnated lung qi,strengthening spleen and drying dampness,promoting urination and diffusing dampness.And he also thinks highly of spleen and stomach protection,which is instructive to clinical application.
