深部热疗联合推拿对膝骨性关节炎患者关节液炎症因子、疗效及足底压力的影响分析

摘　　要：目的:探讨深部热疗联合推拿对膝骨性关节炎(KOA)患者疗效及足底压力、炎症指标的影响。方法:选取2016年1月~2018年12月于河北省沧州中西医结合医院就诊的KOA的患者60例,随机分为观察组、对照组各30例。对照组仅给予推拿治疗,观察组给予深部热疗结合推拿治疗,1次/d,治疗15天。观察比较两组患者治疗前后关节液炎症因子、足底压力及疗效。结果:观察组总有效率优于对照组(P<0.05)。治疗后,两组患者的关节液IL-6、TNF-α、MMP3含量均较治疗前显著降低(P<0.01),且观察组治疗后的关节液IL-6、TNF-α、MMP3含量显著低于对照组(P<0.01)。足底各区域受力时间百分比观察组除M5区外均较治疗前缩短(P<0.05),对照组仅M2、M4、HM区较治疗前缩短(P<0.05),而两组除M1、M5区外均有显著性差异(P<0.05)。结论:单独推拿和深部热疗联合推拿,可显著降低KOA患者关节滑液的炎症因子,缩短足底触地时间比,提高临床治疗疗效,且深部热疗联合推拿效果较佳。Objective: Investigate the effect of deep hyperthermia combined with massage on plantar pressure and inflammation in patients with knee osteoarthritis( KOA). Methods: From January 2016 to December 2018,60 patients w ith KOA w ere selected from our Hospital,w hich w ere randomly divided into observation group and control group in 30 cases each. The control group w as given massage only,and the observation group w as treated w ith deep hyperthermia combined w ith massage,once a day for 15 days. To observe and compare the inflammatory factors,plantar pressure before and after treatment and curative effect of joint fluid. Results: The total effective rate of observation group w as better than that of control group( P <0. 05). After treatment,the levels of IL-6,TNF-α and M M P3 in the tw o groups w ere significantly low er than those before treatment( P < 0. 01),the contents of IL-6,TNF-α and M M P3 w ere significantly low er in the observation group than in the control group( P < 0. 01). The percentage of stress time in each area of the plantar w as shorter than before treatment in the observation group except M5 area( P < 0. 05);Only M2,M4 and HM regions in the control group w ere shorter than those before treatment( P < 0. 05),there w ere significant differences betw een the tw o groups except M1 and M5 regions( P < 0. 05). Conclusion: M assage alone and deep hyperthermia combined w ith massage can significantly reduce the inflammatory factors of synovial fluid in patients w ith knee osteoarthritis,shorten the time ratio of plantar contact w ith the ground,improve the clinical efficacy,and the effect of deep hyperthermia combined w ith massage is better.
