摘　　要：目的了解太原市艾滋病病毒(HIV)抗体筛查阳性样本的确证试验结果,为太原市艾滋病检测工作提供依据。方法对太原市2016~2018年各HIV抗体初筛实验室检测出的HIV抗体阳性样本,采用免疫印迹试验(WB)进行确证,并用SPSS 19.0软件进行统计学分析。结果 2016~2018年3 134份初筛阳性样本中,确证阳性样本2 638份,占84.17%;不确定样本297份,占9.48%;阴性样本199份,占6.35%。不同年份确证阳性、不确定和阴性样本检出比例有差异有统计学意义(P<0.01);HIV感染者以男性(88.51%)、20~39岁(59.09%)者居多;不同样本来源确证检测结果有差异有统计学意义(P<0.01),来自于自愿咨询检测门诊的确证阳性率最高,血液中心的确证阳性率最低;所有的确证阳性样本中,gp160、p66和p24的3个条带出现频率较高。结论近年来,太原市初筛阳性样本的WB确证阳性比例呈上升趋势;通过WB确证实验可以排除筛查实验中的假阳性,对于确证结果不确定的样本应加大随访力度。Objective To understand the confirmatory results of HIV positive samples in Taiyuan city,so as to provide the evidence for AIDS detection in Taiyuan city. Methods The positive samples of HIV antibodies detected by HIV antibody screening laboratories in Taiyuan city during 2016-2018 were confirmed by WB test and analyzed by SPSS19.0 software. Results Of the 3 134 positive samples during 2016-2018,a total of 2 638 samples were confirmed positive,accounting for 84.17%;297 samples were uncertain,accounting for 9.48%;and 199 samples were negative,accounting for 6.35%;there were significant differences in the detection rates of positive,uncertain and negative samples among different years(P<0.01);the majority of HIV-infected persons were male(88.51%)and 20-39 years old(59.09%);the results of WB test from different sample sources were statistically significant different(P<0.01),the positive rate of WB test from voluntary counseling and testing clinic(VCT) was the highest,and that from blood center was the lowest;among all confirmed positive specimens,the occurrence frequency of bands gp160,p66 and p24 was higher. Conclusion In recent years,the proportion of positive samples confirmed by WB in primary screening in Taiyuan city is on the rise.The false positive in the screening test can be excluded by WB confirmation test,at the same time,follow-up efforts should be strengthened for samples with uncertain confirmation results.
