日间手术模式下细银质针导热松解术的临床应用回顾分析

摘　　要：目的:回顾日间手术模式下细银质针导热松解术的有效性和安全性。方法:回顾分析2017年8月至2019年9月在中日医院日间手术中心接受细银质针导热松解术病人的一般情况、病程、病种、平均住院日、平均住院费用、病人满意度、疼痛数字评分(numerical rating scale,NRS)、睡眠质量(athens insomnia scale,AIS)、生活质量和并发症。结果:共分析病人596例,平均病程45.2个月,平均住院日1.02天,平均病人满意度8.06分,平均住院费用1953.95元。病种主要包括筋膜炎374例(62.8%)、关节源性疾病124例(20.8%)、神经病理性疼痛98例(16.4%)。病人治疗1周、4周后的NRS、AIS均较治疗前显著降低(P<0.05)。PCS评分在治疗后1周和4周时较治疗前显著改善(P<0.05),MCS评分在治疗后4周时较治疗前显著改善(P<0.05)。共发生并发症34例,其中皮下瘀血血肿33例,气胸1例。结论:日间手术模式下,细银质针导热松解术效果好,风险低,是慢性骨骼肌肉疼痛的良好治疗方法。Objective:To retro spectively analyze the clinical efficacy and safety of thin silver needle heat conduction in ambulatory surgery mode.Methods:The course of disease,diagnosis type,length of hospital stay,inpatient charges,patients'satisfaction,numerical rating scale(NRS),Athens insomnia scale(AIS),Quality of life scale(SF-12),and complications,if any,in the patients undergoing thin silver needle heat conduction in Ambulatory Surgery Center of China-Japan Friendship Hospital between August 2017 and September 2019 were retro spectively analyzed.Results:A total of 596 patients were retrospectively analyzed,with an average length of hospital stay of 1.02 days,an average satisfaction score of 8.06,and an average inpatient charge of 1953.95 RMB.The average course of disease was 45.2 months,including 374 cases of fasciitis(62.8%),124 cases of articular diseases(20.8%),and 98 cases of neuropathic pain(16.4%).NRS score and AIS score of patients at 1 and 4 weeks after treatment were significantly lower than those before treatment(P<0.05).PCS score was significantly improved at 1 and 4 weeks after treatment(P<0.05),while the MCS score was significantly improved at 4 weeks after treatment(P<0.05).Complications occurred in 34 cases,including 33 cases of subcutaneous hematoma and 1 case of pneumothorax.Conclusion:In ambulatory surgery mode,the thin silver needle heat conduction has a good effect and low risk,and is an excellent treatment for chronic musculoskeletal pain.
