胃造口双管法在外伤性十二指肠破裂修复术中的应用

摘　　要：目的分析外伤性十二指肠破裂患者修复手术中应用胃造口双管法的效果。方法回顾性分析2017年3月至2018年10月本院收治的36例外伤性十二指肠破裂患者的临床资料,按照随机抽样法分为试验组和参考组,各18例。患者均实施十二直肠修复术,试验组患者采用胃造口双管法,参考组患者应用Hassan三管法,对比分析两组患者手术时间、术中出血量、肠内营养管留置时间、住院时间以及感染、十二指肠瘘、肠梗阻等术后并发症发生情况。结果试验组患者手术时间短于参考组,差异有统计学意义(P<0.05),两组患者术中出血量比较差异无统计学意义,试验组患者肠内营养管留置时间以及住院时间均显著短于参考组,差异均有统计学意义(P<0.05)。试验组患者术后并发症发生率为16.67%,参考组为38.89%,比较差异有统计学意义(P<0.05)。结论外伤性十二指肠破裂患者修复手术中应用胃造口双管法不但能够缩短手术时间,同时还可缩短术后恢复时间并且能够有效抑制术后并发症发生率,临床疗效显著。Objective To analyze the effect of gastrostomy double-tube method in repair of patients with traumatic duodenal rupture. Methods The clinical data of 36 patients with traumatic duodenal rupture treated in our hospital from March 2017 to October 2018 were retrospectively analyzed. They were divided into experimental group and reference group according to the random sampling method, with 18 cases in each group. For rectal repair, patients in the experimental group used the double gastrostomy method and patients in the reference group applied the hassan triple tube method. The surgical time,intraoperative blood loss, intestinal nutrition tube indwelling time, hospitalization time, infection, Postoperative complications such as duodenal fistula and intestinal obstruction were compared between two groups. Results The operation time of the experimental group was shorter than that of the reference group, and the difference was statistically significant(P<0.05). There was no significant difference in the amount of intraoperative blood loss between the two groups of patients. And the length of hospital stay in the experimental group was significantly shorter than the reference group, and the differences were statistically significant(P<0.05). The total incidence of postoperative complications in the experimental group was 16.67%, compared with 38.89%in the reference group, and the difference was statistically significant(P<0.05). Conclusion The application of the double gastrostomy method in the repair of patients with traumatic duodenal rupture can not only shorten the operation time, but also shorten the postoperative recovery time and can effectively suppress the incidence of postoperative complications. The clinical effect is significant.
