探讨后外侧结构重建对后外侧入路全髋关节置换术后早期脱位的影响

摘　　要：目的探讨后外侧结构重建对后外侧入路全髋关节置换术后早期脱位的影响。方法选取2016年7月至2019年7月本院收治的需要进行后外侧入路全髋关节置换手术的患者70例作为研究对象,依据是否对后外侧结构进行重建分为研究组与对照组,每组35例。研究组在后外侧入路全髋关节置换手术前对后外侧结构进行重建,对照组在后外侧入路全髋关节置换手术前不对后外侧结构进行重建。对比两组患者的髋关节早期脱位发生率。结果治疗后,对照组的髋关节早期脱位率明显高于研究组,差异具有统计学意义(P<0.05);研究组患者的术中失血量、手术时间、切口长度显著低于对照组,差异具有统计学意义(P<0.05)。结论在后外侧入路全髋关节置换手术中,进行后外侧结构重建的患者较不进行后外侧结构重建的患者术后发生早期的脱位率显著降低,重建后外侧结构可减少后外侧入路全髋关节置换术后早期脱位的发生,对患者的手术效果、术后恢复及对髋关节软组织的平衡具有重要作用,值得临床推广应用。Objective To analysis the posterolateral structure reconstruction of posterolateral approach, the influence of early dislocation after total hip replacement. Methods From July 2016 to July 2019, 70 patients who needed to undergo total hip replacement via posterolateral approach were selected as the study subjects. According to whether to reconstruct the posterolateral structure, they were divided into study group and control group, with 35 cases in each group. In the study group, posterolateral structures were reconstructed before the total hip arthroplasty through posterolateral approach, while in the control group, posterolateral structures were not reconstructed before the total hip arthroplasty through posterolateral approach. The incidence of early dislocation of hip was compared between the two groups. Results After treatment, early hip dislocation rate of control group was obviously higher than that of study group, the difference was statistically significant(P<0.05). Intraoperative blood loss, operative time, length of incision in study group was significantly lower than in the control group, the difference was statistically significant(P<0.05). Conclusion The posterolateral approach in the total hip replacement surgery, posterolateral structure reconstruction of patients than posterolateral structure reconstruction in patients with postoperative early dislocation rate significantly reduced. Reconstruction of the posterolateral structure can reduce the posterolateral approach the occurrence of early dislocation after total hip replacement, effect of surgery, postoperative recovery of patients and plays an important role in the hip joint soft tissue balance, it is worthly applied in application.
