不典型膜性肾病和膜性肾病的临床及病理分析

摘　　要：目的探究不典型膜性肾病和膜性肾病的临床及病理特点。方法选取本院2014年11月至2018年11月收治的43例不典型膜性肾病(A组)和174例特发性膜性肾病患者(B组)作为研究对象,分析两组患者临床及病理特点。结果两组临床资料中仅年龄、合并肾病综合征对比差异有统计学意义(P<0.05),其他资料对比差异均无统计学意义。两组尿RBC、24 h尿蛋白定量、血清白蛋白对比差异有统计学意义(P<0.05);其他实验室指标对比差异均无统计学意义。两组肾小管间质急性病变以及肾小管间质慢性病变严重程度对比差异有统计学意义(P<0.05)。两组IgA、IgM、C4、C1q阳性率对比差异有统计学意义(P<0.05);两组IgG、C3阳性率对比差异无统计学意义。结论不典型膜性肾病和特发性膜性肾病在临床及病理特点方面差异明显,临床中应注意对两者的鉴别诊断,制定合理的治疗计划,从而改善患者预后。Objective To investigate the clinical and pathological features of atypical membranous nephropathy and membranous nephropathy.Methods 43 cases of atypical membranous nephropathy(group A) and 174 cases of idiopathic membranous nephropathy(group B) admitted to our hospital from November 2014 to November 2018 were selected as the research subjects, and the clinical and pathological characteristics of the two groups of patients were analyzed. Results There were only statistically significant differences in age and combined nephrotic syndrome between the two groups(P<0.05), but no statistically significant differences in other data. There were statistically significant differences between the two groups in urine RBC, urine protein quantification and serum albumin at 24 h(P<0.05). There was no statistically significant difference between other laboratory indexes and other data. There were statistically significant differences in the severity of acute tubulointerstitial lesions and chronic tubulointerstitial lesions between the two groups(P<0.05). The positive rates of IgA, IgM, C4 and C1 q in the two groups were statistically significant(P<0.05).There was no significant difference in IgG and C3 positive rate between the two groups. Conclusion The clinical and pathological features of atypical membranous nephropathy and idiopathic membranous nephropathy are different, so should pay attention to the differential diagnosis of them and make a reasonable treatment plan, so as to improve the prognosis of patients.
