摘 要:目的探讨经阴道三维输卵管超声造影(TVS-3D-HyCoSy)检查中,三种不同子宫位置对输卵管显示的影响,并探讨经腹部输卵管超声造影(TAS-HyCoSy)检查对成像质量的影响。方法回顾性分析244例行HyCoSy检查患者图像。将子宫位置分为前位、平位、后位。将输卵管分为近段、远段,根据各段的显示情况,分为显示、未显示。分析子宫位置对输卵管近、远段显示的影响。对于TVS-3D-HyCoSy中未全程显示的输卵管,评价加行TAS-HyCoSy对其成像的影响。结果244例患者(431条输卵管)纳入本研究。TVS-3D-HyCoSy下,平位子宫对应的输卵管近段未显示的风险增加(OR:4.586,95%CI:2.499~8.415),子宫位置对输卵管远段显示的影响无显著性差异(P>0.05)。对48条TVS-3D-HyCoSy下未全程显示的输卵管加行TAS-HyCoSy,45条输卵管全程显示。结论对于平位子宫,当TVS-3D-HyCoSy下输卵管近段未显示时,可行TAS-HyCoSy改善其显示情况,提高输卵管的显示率。Objective To identify the effect of uterine position on the fallopian tube display in transvaginal three-dimensional hysterosalpingo-contrast sonography(TVS-3 D-HyCoSy)and the effect of transabdominal hysterosalpingo-contrast sonography(TAS-HyCoSy)on the improvment of display quality.Methods 244 patients examined with TVS-3 D-HyCoSy were included.The uterine position was divided into anteposition,horizontal position,and retroposition.Dividing the fallopian tube into the proximal segment and the distal segment.According to the display of the fallopian tube segments,they were divided into visible and invisible.Identify the effect of uterine position on the display of each segment of fallopian tube in TVS-3 D-HyCoSy.In addition,evaluate the effect of TAS-HyCoSy on the improvment of display quality.Results 431 fallopian tubes were included in the study.Horizontal position of uterus was the risk factor of invisible of the proximal segment of fallopian tube(OR:4.586,95%CI:2.499-8.415).While the display of distal segment of fallopian tube was not influenced by uterine position.45 fallopian tubes which could not be fully shown in TVS-3 D-HyCoSy were fully shown in TAS-HyCoSy.Conclusions For the horizontal position uterus,it′s necessary to consider the possibility of false positives caused by poor display quality of the sonogram when the fallopian tube was invisible in TVS-3 D-HyCoSy.In addition,TAS-HyCoSy should be a good choice to improve the display quality and increase the diagnostic accuracy.
