彩超与CT对浅表淋巴结结核诊断价值的对比研究

摘　　要：目的探讨彩色多普勒超声与常规CT应用于浅表淋巴结结核中的诊断价值。方法选取100例疑似浅表淋巴结结核患者作为研究对象,分别给予常规CT、彩色多普勒超声检查。最终检查结果以医院手术组织标本病理检查为准,并将检查结果进行比较和分析,比较两组诊断准确度和特异度、灵敏度、阳性预测值及阴性预测值,观察不同类型浅表淋巴结结核采用彩色多普勒超声检查表现情况。结果彩色多普勒超声检查准确度和特异度、灵敏度、阳性预测值及阴性预测值均明显高于常规CT检查。彩色多普勒超声检查表现:干酪坏死型表现为类圆形或不规则,存在不均质的极低回声,血流稀疏或消失;淋巴结炎症型表现为长椭圆形或类圆形,皮质增厚、回声减低、血流增多;液化脓肿型患者则主要表现为体积增大,且其内部回声表现为不均匀且杂乱,同时可见液化、回声减弱等。结论临床采用彩色多普勒超声检查诊断浅表淋巴结结核效果优于常规CT检查,对临床诊治方案制定提供有力参考依据,可有利于缩短患者病程,减轻其痛苦,促进其康复。Objective To explore the diagnostic value of color Doppler ultrasound and conventional CT in superficial lymph node tuberculosis.Methods 100 patients with suspected superficial lymph node tuberculosis admitted to the hospital were selected as the subjects of this study,and were given conventional CT and color Doppler ultrasonography examinations.The results of laboratory pathological biopsy were used as the gold standard.The results were compared and amalyzed.The diagnostic accuracy and specificity,sensitivity,positive predictive value and negative predictive value of the two groups were compared.The performance of different types of superficial lymph node tuberculosis was observed by color Doppler ultrasonography.Results The accuracy and specificity,sensitivity,positive predictive value and negative predictive value of color Doppler wleras onagraphy were significantly higher than thot of conventional CT examination.Color Doppler ultrasonography momifestations:cheese necrosis showed circular or irregular,very low echo of heterogeneity,sparse or disappeared blood flow;lymph node inflammation showed long oval or round,cortical increase thickness,echo reduction,increased blood flow;liquefied abscess type showed irregular volume increase,internal echo performance was disordered,uneven,visible liquefaction and weak echo.Conclusions The application of color Doppler ultrasonography in diagnosis of superficial lymph node tuberculosis is superior to conventional CT examination,providing a strong reference for clinical diagnosis and treatment plan,which can shorten the couse of patients,reduce their pain and promote their recovery.
