某眼科医院中药注射剂药物利用的研究

摘　　要：目的分析中药注射剂(TCMIs)在某眼科医院中的利用情况,为其合理使用提供参考。方法收集某三甲眼科医院信息管理系统提供的2014―2018年全部TCMIs的用药数据,进行用药频度(DDDs)分析。随机抽取2014―2018年使用TCMIs的住院病历400份,进行药物利用评价,计算药物利用指数(DUI),药物不合理使用因素的不合理率。结果2014―2018年眼科TCMIs销售金额和DDDs均呈先上升后下降的趋势;DDDs最高的是注射用灯盏花素;除红花注射液和银杏达莫注射液外的其他12种TCMIs具有良好的经济性与社会效益的同步性;不同规格的银杏达莫注射液和注射用丹参多酚酸盐日均费用(DDC)相差较大。TCMIs平均DUI为0.93,TCMIs运用不合理因素主要为无中医辨证用药、适应证不适宜和给药频次不适宜。结论TCMIs在眼科医院的应用较广,医院须优选个别药品的规格从而节约药品费用,加强医护人员中医药理论的培训,严格辨证用药,按照说明书推荐用药,促进TCMIs的合理应用。Objective To analyze the utilization of the traditional Chinese medicine injections(TCMIs)in an ophthalmic hospital,and provide references for the rational use of TCMIs.Methods The using data of all TCMIs from 2014 to 2018 provided by hospital information system were collected and to carry out defined daily doses(DDDs)analysis.A total of 400 hospitalized medical records of TCMIs used from 2014 to 2018 were randomly selected for drug utilization evaluation,and the drug utilization index(DUI)and unreasonable rate of unreasonable drug use factors were calculated.Results From 2014 to 2018,the sales amount and DDDs of TCMIs showed a trend of first rising and then declining;the Breviscapine injection had the highest DDDs;except Safflower injection and Ginkgo-damole injection,other 12 TCMIs showed good synchronism of economic and social benefits;the defined daily cost(DDC)of the different specifications Ginkgo-damole injection and Danshen polyphenolate for injection were quite different.The average DUI of TCMIs was 0.93,and the mainly unreasonable factors of TCMIs application were no TCM syndrome differentiation,unsuitable indications and inappropriate frequency of administration.Conclusion TCMIs is widely used in ophthalmic hospital.Hospital should choose suitable product specifications so as to save the cost of drugs,strengthen the training of traditional Chinese medicine theory of medical staff,strictly carry out dialectical treatment,recommend drugs according to the instructions,and promote the rational application of TCMIs.
