摘 要:目的探究盐酸氨溴索对呼吸机相关性肺炎(VAP)患者感染控制、氧合指数及预后的影响。方法选取2018年1月~2018年10月于我院接受治疗的78例VAP患者为受试对象,按照入院顺序随机分为研究组与对照组各39例。两组患者均采用氧疗及营养支持等常规对症支持治疗,研究组在其基础上给予静脉泵注注射用盐酸氨溴索。比较两组患者疗效及治疗期间不良反应情况,记录治疗前及治疗2周后炎症因子(白细胞计数、中性粒细胞分类)水平、肺部感染(CPIS)评分、氧合指数。结果研究组患者治疗有效率高于对照组患者(P<0.05),治疗2周后,两组患者的白细胞计数、中性粒细胞分类及CPIS水平较治疗前均有下降,且研究组下降幅度大于对照组(P均<0.05);两组患者氧合指数较治疗前均有上升,且研究组上升幅度大于对照组(P<0.05)。结论盐酸氨溴索可加大对VAP患者的感染控制力度和氧合指数,改善预后。Objective To explore the effects of ambroxol hydrochloride on infection control,oxygenation index and prognosis in patients with ventilator-associated pneumonia(VAP).Methods 78 patients with VAP who were treated in our hospital from January 2018 to October 2018 were selected for the study and were randomly divided into study group and control group according to the order of admission,with 39 cases in each group.The two groups were given conventional symptomatic support such as oxygen therapy and nutritional support,and study group was given intravenous pump injection of ambroxol hydrochloride on this basis.The efficacy and adverse reactions during treatment were compared between the two groups.The levels of inflammatory factors(white blood cell count,neutrophil classification),pulmonary infection(CPIS)score and oxygenation index were recorded before treatment and after 2 weeks of treatment.Results The effective rate of treatment in study group was higher than that in control group(P<0.05).After 2 weeks of treatment,the white blood cell count,neutrophil classification and CPIS level in the two groups were decreased compared with those before treatment,and the decreases in study group were greater than those in control group(all P<0.05).The oxygenation index in the two groups was increased compared with that before treatment,and the increase in study group was greater than that in control group(P<0.05).Conclusion Ambroxol hydrochloride can increase the infection control intensity and oxygenation index and improve the prognosis of patients with VAP.
