摘　　要：目的了解年龄对孕产妇的不良分娩结局的影响。方法采用回顾性的研究方法,收集河北省2013-2017年期间在河北省危重孕产妇监测系统监测的22所医院入院分娩的孕产妇资料;以25~29岁组作为对照组,通过校正混杂因素后的多因素logistic回归方法分析不同年龄段孕产妇的不良分娩结局。结果共纳入262 924例孕产妇,平均生育年龄(28.00±4.54)岁,整体呈逐年上升趋势;30岁以后发生妊娠期糖尿病的风险随着年龄增长而增加(P<0.001);子痫前期是高龄妊娠(>35岁)的危险因素,OR值(95%CI)为1.966(1.668~2.316);早产、低出生体重及新生儿死亡在低龄妊娠和高龄妊娠发生的风险均较高,低龄妊娠(<20岁)OR值(95%CI)值分别为;早产:1.764(1.538~2.023)、低出生体重:2.170(1.877~2.495)、新生儿死亡:2.399(1.673~3.439);高龄妊娠(≥40岁)OR值(95%CI)分别为早产:2.008(1.785~2.258)、低出生体重:1.961(1.706~2.255)、新生儿死亡:3.229(2.277~4.580)。结论低龄妊娠和高龄妊娠均会发生不良分娩结局,鼓励女性适龄期生育。Objective To analyze maternal and perinatal outcomes of pregnant women in different age. Methods A retrospective study was conducted to collect the data from 22 hospitals in Hebei Maternal Near Miss Surveillance System from 2013 to 2017.The 25-29 years old group was the control group, after adjusting confounding factors, the adverse maternal and perinatal outcomes of women in other age groups were analyzed by multivariate logistic regression analysis. Results A total of 262,924 pregnant women were included, with an average age of delivery(28.00 ± 4.54) years, and the overall trend was increasing by year.The risk of gestational diabetes after age 30 increases with age(P<0.001);pre-eclampsia was risk factors for advanced pregnancy(>35 years), and the associated ORs(95% CI) was 1.966(1.668-2.316). Preterm, low birth weight and stillbirth are associated with higher risk of pregnancy in the lower and advanced pregnancies, the associated ORs(95% CI) of the younger pregnancy were 1.764(1.538-2.023), 2.170(1.877-2.495), 2.399(1.673-3.439), respectively;the associated ORs(95% CI) of the advanced pregnancy were2.008(1.785-2.258), 1.961(1.706-2.255), and 3.229(2.277-4.580), respectively. Conclusion Adverse delivery outcomes occur in both younger and advanced pregnancies,and women are encouraged to give birth at an appropriate age.
