摘 要:目的:通过熏蒸疗法对KOA家兔滑膜组织治疗前后的病理学形态变化及其相关因子炎性表达的分析,探讨苗药验方骨炎消对膝关节滑膜炎的治疗作用。方法:采用膝关节腔内注射碘乙酸钠的方法建立家兔膝骨性关节炎的炎症模型,正常组和模型组正常饲养,不做干预;阳性组施予扶他林软膏外用;早期苗熏组为造模后第一天给予苗药熏蒸治疗;苗药熏蒸组为造模2周后给予熏蒸治疗。固定兔膝关节,熏蒸30min,1次/d,持续6周。治疗结束后取家兔膝关节滑膜组织进行HE染色观测其病理学形态变化;免疫组织化学法检测滑膜组织中白细胞介素1β(IL-1β)和肿瘤坏死因子α(TNF-α)的炎性表达;蛋白免疫印迹法(Western-blot)检测NF-κB和p38的蛋白表达水平。结果:与模型组相比,光镜下观察其余各组治疗后的滑膜组织,其病理学形态均有明显改善;与正常组相比,模型组家兔滑膜组织IL-1β与TNF-α的炎性表达显著升高,且NF-κB和p38蛋白的表达水平显著升高,差异均具有统计学意义(P<0.05);与模型组相比,阳性组、早期苗熏组和苗药熏蒸组的IL-1β与TNF-α的炎性表达明显降低,且NF-κB和p38蛋白的表达水平明显降低,差异均具有统计学意义(P<0.05)。结论:熏蒸疗法可使验方骨炎消改善KOA家兔滑膜组织的病理学形态,且有效抑制滑膜组织中相关因子的炎性表达,从而减轻炎症反应,延缓膝骨性关节炎的发病进程,具有一定的运用和推广价值。Objective:To investigate the therapeutic effect of Guyanxiao on knee synovitis by fumigation therapy on pathological changes and inflammatory expression of related factors in KOA rabbit synovial tissue before and after treatment.Methods:Inflammatory model of knee osteoarthritis in rabbits was established by injecting sodium iodoacetate into knee joint cavity.Normal group and model group were fed normally without intervention.Positive group was given Votalin ointment for external use;early seedling fumigation group was given Miao medicine fumigation treatment on the first day after modeling;Miao medicine fumigation group was given fumigation treatment after two weeks of modeling.The knee joint of rabbit was fixed and fumigated for 30minutes once a day for 6weeks.After treatment,the synovial tissue of knee joint of rabbits was stained with HE to observe its pathological changes;the inflammatory expression of interleukin-1beta(IL-1beta)and tumor necrosis factor-alpha(TNF-alpha)in synovial tissue was detected by immunohistochemistry;the protein expression of NF-kappa B and p38was detected by Western-blot.Results:Compared with the model group,the pathological morphology of synovial tissue in the other groups was improved significantly after treatment under light microscope,and the pathological morphology of each group was significantly improved.Compared with normal group,the inflammatory expression of IL-1βand TNF-αin synovial tissue of model group was significantly higher,and the expression levels of NF-κB and p38protein were significantly higher,the difference was statistically significant(P<0.05).The inflammatory expression of IL-1beta and TNF-αin early seedling fumigation group and seedling fumigation group were significantly decreased,and the expression levels of NF-κB and p38protein were significantly decreased,the difference was statistically significant(P<0.05).Conclusion:Fumigation therapy can improve the pathological morphology of synovial tissue of KOA rabbits and effectively inhibit the expression of
