喉罩下七氟醚吸入全身麻醉在剖宫产术中的临床应用

摘　　要：目的:分析喉罩下七氟醚吸入全身麻醉在剖宫产术中的临床应用情况。方法:选定2018年1月~2019年3月本院收治的剖宫产产妇82例,对产妇进行随机编号,采取抽签法进行分组,对照组(41例,气管插管全身麻醉)、试验组(41例,喉罩下七氟醚吸入全身麻醉),比较两组各个时段产妇血压、心率情况,分析新生儿娩出情况。结果:入室时两组患者心率(HR)、平均动脉压(MAP)组间比较差异较小,入喉罩(插管)时、切皮时、拔喉罩(拔管)时两组上述指标均较入室时升高,P<0.05,但试验组低于对照组,P<0.05;试验组产妇苏醒时间短于对照组,P<0.05,但新生儿Apgar评分与对照组相比差异较小,P>0.05。结论:在剖宫产手术中,喉罩下七氟醚吸入全身麻醉较气管插管全身麻醉对产妇血流动力学指标影响较小,利于胎儿娩出,应用价值更高。Objective:To analyze the clinical application of sevoflurane inhalation general anesthesia under the laryngeal mask in cesarean section.Methods:82 cases of cesarean women who were admitted to our hospital from January 2018 to March 2019 were selected.The women were randomly numbered and grouped by lottery.The control group(41 cases,general anesthesia with tracheal intubation),trial Group(41 cases,general anesthesia with sevoflurane inhalation under the laryngeal mask),compared the maternal blood pressure and heart rate of the two groups at different periods,and analyzed the neonatal delivery.Results:The differences in heart rate(HR)and mean arterial pressure(MAP)between the two groups were small at the time of admission.The above indexes were obtained when the laryngeal mask was intubated,the skin was cut,and the laryngeal mask was removed.They were all higher than those at the time of admission,P<0.05,but the test group was lower than the control group,P<0.05;the maternal wake-up time in the test group was shorter than that in the control group,P<0.05,but the difference in Apgar score of the newborn was smaller than that in the control group,P>0.05.Conclusion:In cesarean section,general anesthesia with sevoflurane inhalation under the laryngeal mask has less effect on maternal hemodynamic parameters than intubation general anesthesia,which is conducive to fetal delivery and has higher application value.
