摘　　要：分析了社区居家医养结合养老模式的涵义、特点和发展困境,以及医务社会工作介入社区居家医养结合养老模式的必要性与可行性,并提出了强化专业服务、优化参与路径、整合多方资源等介入策略。The purpose of this paper is to analyze the meaning,characteristics and development dilemma of the mode of community home-based for the aged in integrated treatment and convalesce mode,as well as the necessity and feasibility of social work involving community home-based for the aged in integrated treatment and convalesce mode. It puts forward some intervention strategies of strengthening professional service,optimizing participation path,integrating resource in many ways and so on.
