哮喘-慢阻肺重叠综合征患者外周血Th17和Treg细胞水平表达与肺功能的关系研究

摘　　要：目的分析哮喘-慢阻肺重叠综合征(ACOS)患者辅助性T细胞17(Th17)和调节性T细胞(Treg)表达水平,及其与肺功能的相关性。方法采用流式细胞术检测60例ACOS患者(ACOS组)、60例哮喘患者(哮喘组)和60例慢性阻塞性肺疾病患者(COPD组)外周血Th17和Treg细胞的表达,比较分析其与肺功能指标的关系。结果与哮喘组比较,COPD组、ACOS组外周血CD4+细胞中Th17细胞表达及Th17/Treg比例明显升高,而Treg细胞表达明显下降,差异均有统计学意义(F=15.36~36.61,均P<0.05);与COPD组比较,ACOS组外周血CD4+细胞中Th17细胞表达及Th17/Treg比例明显升高,而Treg细胞表达明显下降,差异有统计学意义(F=36.61,21.18,15.26, P<0.05)。ACOS组患者FVC%pred,FEV1%pred和FEV1/FVC明显低于哮喘组,差异均有统计学意义(F=7.36,11.36,9.68,P<0.05),但FVC%pred与COPD组患者比较,差异无统计意义(t=0.52,P>0.05)。ACOS患者外周血Th17和h17/Treg表达与FVC%pred,FEV1%pred和FEV1/FVC呈显著负相关(r=-0.502^-0.407,均P<0.05)。Treg与FVC%pred,FEV1%pred和EV1/FVC呈显著正相关(r=0.019~0.033,均P<0.05)。结论ACOS患者外周血Th17和Treg细胞表达异常,存在Th17/Treg比例失衡,且与肺功能下降密切相关。Objective To analyze the expression levels of T help cell 17(Th17), T regular cell(Treg) in patients with asthma-COPD overlap syndrome(ACOS), and their correlation with lung function. Methods Flow cytometry was used to detect the expression of Th17 and Treg cells in peripheral blood of 60 ACOS patients(ACOS group), 60 asthma patients(asthma group) and 60 patients with chronic obstructive pulmfonary disease(COPD group), and the relationship between expression of Th17 and Treg lung function indexes was compared and analyzed. Results Compared with the asthma group, the expression of Th17 cells and the proportion of Th17/Treg in peripheral blood CD4+ cells in the COPD group and ACOS group were significantly increased, while the expression of Treg cells was significantly decreased,the difference was statistically significant(F=15.26~3661,all P<0.05). Compared with the COPD group, the expression of Th17 cells and the proportion of Th17/Treg in the ACOS group s peripheral blood CD4+ cells were significantly increased, while the expression of Treg cells was significantly decreased, the difference was statistically significant(F=36.61,21.18,15.26, P<0.05). FVC%pred, FEV1% pred and FEV1/FVC in ACOS group were significantly lower than those in asthma group,the difference was statistically significant(F=7.36,11.36,9.68, P<0.05), but there was no significant difference between FVC%pred and COPD group(t=0.52, P>0.05). The expression of Th17 and Th17/Treg in the peripheral blood of ACOS patients was negatively correlated with FVC%pred, FEV1% pred and FEV1/FVC(r=-0.50 2^-0.407, all P<0.05). Treg was significantly positively correlated with FVC%pred, FEV1% pred, and FEV1/FVC(r=0.019~0.033, all P<0.05). Conclusion There was abnormal expression of Th17 and Treg cells in peripheral blood of patients with ACOS, and resulting in imbalance of Th17/Treg ratio, which was closely related to reduction of lung function.
