摘 要:目的应用分层斑点追踪(LS-STE)评估正常小鼠和心力衰竭模型小鼠左室内膜下和外膜下心肌的应变差异及应变参数与心肌纤维化的相关性。方法正常雄性健康小鼠,随机分为两组,一组行主动脉缩窄术,另一组作为假手术组。术后第3周,应用常规超声和分层斑点追踪技术评估小鼠的心功能。小鼠心肌纤维损伤的严重程度采用Masson-some染色法评估。心肌纤维损伤程度与LS-STE所测指标的关系,采用Pearson相关性分析进行评估。结果常规超声学指标提示手术组小鼠具有明显心衰特征。LS-STE指标提示内膜下、外膜下应变及室壁最大达峰延迟时间(MOWD)在假手术组和手术组小鼠间存在显著差异(均P<0.05);在假手术组和手术组小鼠中,内膜下应变值在纵向和环向均明显高于外膜下应变(均P<0.05);相关性分析提示LS-STE所测指标与小鼠心肌纤维化程度显著相关。结论LS-STE能稳定可靠地评估小鼠左心室内、外膜下心肌的应变,为小鼠心血管疾病模型的进一步研究提供了稳定可靠的参考值。Objective To evaluate left ventricular endocardium and epicardium strain in normal and heart failure mouse models using layer specific-speckle tracking echocardiography(LS-STE)and the correlation between the strain parameters and myocardium fibrosis.Methods Normal male mice were selected randomly and divided into 2 groups.One group was subjected to aortic banding and the other served as sham group.Three weeks after the surgery,conventional echocardiography and LS-STE were applied to evaluate the cardiac function of mice.The fibrotic lesion of left ventricular was analyzed using Masson-some trichrome staining.The correlations between the extent of fibrotic lesion and strain parameter were evaluated.Results The conventional echocardiographic parameters in the operation group showed the typical signs of heart failure.There were significant difference in LS-STE parameters such as endocardium strain,epicardium strain and maximal opposite wall delay(MOWD)between the operation group and the sham group(P<0.05).Endocardium strain was significantly higher both in the sham group and the operation group than in the epicardium strain(P<0.05).Pearson correlation analysis indicated that LS-STE parameters were significantly correlated with fibrotic lesion(P<0.05).Conclusion LS-STE technique can assess left ventricular epicardial and endocardial myocardium strain alternation reliably and stably.Our research provides normal reference value for further research in the mice model of cardiovascular disease.
