摘　　要：目的评价结核感染T细胞斑点试验(T-spot.TB)在艾滋病(AIDS)人群中诊断潜伏结核感染的应用价值,了解AIDS患者中潜伏结核感染情况。方法应用T-spot.TB试剂盒对120例AIDS患者的外周血液标本进行检测,并同时进行结核菌素纯蛋白衍生物(PPD)试验。结果在总体AIDS患者、CD4^+T淋巴细胞计数水平(简称CD4细胞)<200个/μL和≥200个/μL组中,T-spot.TB阳性率为30.8%、32.7%和27.3%,PPD检测阳性率分别为13.3%、9.2%和20.5%。同时,进一步分析发现T-spot.TB检测率与患者免疫状态无关。结论T-spot.TB检测AIDS合并潜伏结核感染阳性率高于PPD试验,T-spot.TB检测对AIDS患者中潜伏结核感染的诊断有较大的应用价值。Objective In order to evaluate the clinical value of T-spot.TB in diagnosis of acquired immune deficiency syndrome(AIDS)patients with latent tuberculosis infection and to understand the infection of tuberculosis among AIDS patients.Methods T-spot.TB kit was used to detect peripheral blood samples of 120 AIDS patients,and the tuberculin pure protein derivative(PPD)test was performed.Results In AIDS patients in general,CD4^+T lymphocyte count level(CD4 cells)<200/μL and greater than 200/μL group,the positive rate of t-spot.TB were 30.8%,32.9%and 27.3%,the positive rate of PPD detection were 13.3%,9.2%and 20.5%.At the same time,further analysis revealed that T-spot.TB detection rate was independent of the patient’s immune status.Conclusion T-spot.TB detection the positive rate of AIDS combined with latent tuberculosis infection is higher than that of PPD test,it has great value for the diagnosis of latent tuberculosis infection in AIDS patients.
