光动力疗法治疗尖锐湿疣照光方法与疼痛的关系分析

摘　　要：目的观察5-氨基酮戊酸光动力疗法(5-ALA-PDT)治疗尖锐湿疣(CA)过程中2种照光方法与疼痛的关系,在保持最佳疗效前提下以探讨最适照光方法。方法将接受光动力治疗的CA患者随机分为A、B 2组,用多功能电离子手术治疗仪去除显性疣体后进行5-ALA-PDT治疗,照光功率分别为80 mW/cm^2×30 min与100 mW/cm^2×24min,每周1次,共3次。照光过程中采用视觉模拟评分法对患者疼痛进行评分并进行比较,观察治疗结束后1个月、6个月的复发率。结果A组第1个月、6个月的复发率分别为7.69%和10.26%,与B组(2.70%和8.11%)相近(均P>0.05)。照光1 min时,A组与B组疼痛评分差异无统计学意义,照光5~10 min疼痛评分比较,A组均显著低于B组差异有统计学意义(均P<0.05)。结论5-ALA-PDT治疗CA照光功率80 mW/cm^2×30 min的疗效与100 mW/cm^2×24min相当,但疼痛不良反应相对较轻。因此,5-ALA-PDT治疗CA照光功率为80 mW/cm^2时长30 min,既能达到良好疗效又能将疼痛的不良反应降到最低,是治疗CA的适宜照光方法。Objective In order to observe the relationship between light dose and pain in the photo-dynamic therapy treatment of condyloma acuminate(CA),and to obtain the appropriate light dose on the premise of maintaining good efficacy.Methods Patients with CA that treated with 5-ALA-PDT were randomly divided into group A and B,the treatment of 5-ALA-PDT was performed after the removal of the dominant wart with multi-function electronic operation.The light power was80 mW/cm^2×30 min,100 W/cm^2×24 min,and the treatment was performed once per week,a total of three times.In the course of treatment,visual simulated scoring method was used to score the patients’pain,and the recurrence rates were observed at first month and sixth month after the treatment.Results The recurrence rates of group A in the first month and sixth month were 7.69%and 10.26%respectively,similar to group B(2.70%and 8.11%)(all P>0.05).When lighting one minute,the pain scores of group A and group B were not significantly different.When lighting 5 to 10 minutes,the pain scores of group A were significantly lower than those of group B,and the differences were statistically significant(all P<0.05).Conclusion In the photo-dynamic therapy treatment of CA,the clinical curative effect of 80 mW/cm^2 duration 30 min is the same as that of100 mW/cm^2 duration 24 min,but the pain side effects are relatively mild.Therefore,5-ALA-PDT treatment of CA has a lighting power of 80 mW/cm^2 for 30 minutes,which can not only achieve good efficacy but also minimize pain side effects,and is the appropriate lighting power for photo-dynamic therapy treatment of CA.
