摘 要:目的探讨IgA肾病(IgAN)牛津分型与进展危险因素及缺氧诱导因子-1α(HIF-1α)之间的关系。方法回顾性分析明确诊断为IgAN的98例患者,收集临床资料及病理资料,病理资料进行牛津分型,分别比较以牛津分型MESTC不同分值分组的血清HIF-1α水平及与肾脏预后相关的临床指标的差异。结果在牛津分型E评分分组中,HIF-1α及24 h尿蛋白定量在E1组高于E0组(P<0.05);在T评分分组中、HIF-1α,年龄、血肌酐及24 h、尿蛋白定量2组比较差异有统计学意义(P<0.05);在C评分分组中,HIF-1α、血肌酐及24 h、尿蛋白定量2组比较差异有统计学意义(P<0.05)。结论研究证实了肾脏病理上牛津分型T评分及C评分与血肌酐及24h尿蛋白定量等IgAN临床进展危险因素有关,而血清HIF-1α在不同牛津分型评分(T,C,E评分)分组中差异显著,提示HIF-1α在病理水平上与IgAN进展密切相关。Objective To explore the relationship among Oxford classification of IgA nephropathy(IgAN)with risk factors of progression and hypoxia inducible factor-1α(HIF-1α).Methods A retrospective analysis of 98 patients who were diagnosed with IgAN was performed.Clinical data and pathological data were collected,and pathological data were classified by Oxford Classification.The differences of serum HIF-1α levels and clinical indexes related to renal prognosis in different score groups of mestc in Oxford were compared.Results In the Oxford Classification E-score group,HIF-1αand 24h urine protein were higher in the E1 group than in the E0 group(P<0.05).In the T-score group,HIF-1α,age,serum creatinine,and 24h urine protein were quantified in the two groups.There was a significant difference between two groups(P<0.05).In the C-score group,there were significant differences of HIF-1α,serum creatinine,and 24h urine protein between the two groups(P<0.05).Conclusion The Oxford classification T score and C score on renal pathology are related to risk factors for clinical progress of IgAN,including serum creatinine and 24h urinary protein quantification.The serum HIF-1αis significantly different in different Oxford classification scores(T,C,E score),suggesting that HIF-1αis closely related to IgAN progression at the pathological level.
