摘　　要：目的探讨鼻塞鼻罩交替法对预防早产儿经鼻持续气道正压通气(NCPAP)所导致鼻部损伤的效果。方法选取新生儿科2018年6月—2019年12月收治的使用NCPAP治疗的早产儿90例,按照随机数字表法分为鼻塞组(n=30),鼻罩组(n=30)及鼻塞鼻罩交替组(n=30),3组分别选择鼻罩、鼻塞及鼻塞鼻罩交替作为无创通气连接界面,观察3组早产儿鼻损伤的发生率及通气效果相关指标。结果3组患儿鼻部损伤发生率差异有统计学意义(P<0.05),交替组鼻部损伤发生率(10.00%)低于鼻塞组(33.33%)和鼻罩组(36.67%)。3组NCPAP治疗时间及NCPAP治疗12~24 h后动脉血气氧分压(PaO2)、二氧化碳分压(PaCO2)及pH值比较,差异均无统计学意义(P>0.05)。结论鼻塞鼻罩交替法可有效地减少早产儿NCPAP所导致的鼻部损伤。Objective To investigate the effect of nasal prongs and nasal mask replacement in the prevention of nasal injury caused by nasal continuous positive airway pressure(NCPAP)in premature infants.Methods Ninety premature infants treated with NCPAP were selected and divided into nasal prongs group(n=30),nasal mask group(n=30)and nasal prongs and nasal mask replacement group(n=30)according to random number table.Nasal mask,nasal prongs,nasal prongs and nasal mask replacement were selected as noninvasive ventilation connection interface in the three groups,respectively.The incidence rate of nasal injury in the three groups was observed.Results There were 10 cases(33.33%)of nasal injury in nasal prongs group,11 cases(36.67%)of nasal injury in nasal mask group and 3 cases(10.00%)of nasal injury in nasal prongs and nasal mask replacement group,with a significant difference(P<0.05).There was no significant difference in NCPAP ventilation time,arterial partial pressure of oxygen(PaO2),partial pressure of carbon dioxide(PaCO2)and pH value after 12-24 hours of NCPAP treatment among the three groups(P>0.05).Conclusion The nasal prongs and nasal mask replacement can effectively reduce the risk of nasal injury caused by NCPAP in premature infants.
