老年2型糖尿病微血管病变与内皮素、基质金属蛋白酶-9及其他危险因素的相关性分析

摘　　要：目的探讨内皮素(ET)、基质金属蛋白酶-9(MMP-9)与老年2型糖尿病(T2DM)患者微血管病变的相关性,并分析影响患者出现微血管病变的危险因素。方法选取2017年8月—2019年3月笔者所在医院治疗的T2DM患者153例作为研究对象,按照微血管病变的有无将其分为微血管病变组(n=85)和无血管病变组(n=68)。比较两组ET、MMP-9水平;分析患者ET、MMP-9含量间的相关性;采用多元logistic回归分析影响患者出现微血管病变的危险因素。结果微血管病变组血清ET、MMP-9水平均高于无血管病变组(P<0.05);患者ET水平与MMP-9水平呈正相关(r=0.469,P=0.000);经多元logistic回归分析得出,患高血压、心脏病、脑卒中、T2DM病程≥10年、血清ET水平≥100 ng/L、血浆MMP-9≥250 ng/ml是影响T2DM患者出现微血管病变的危险因素(P<0.05)。结论T2DM患者ET、MMP-9水平与其出现微血管病变有关,且患高血压、心脏病、脑卒中、T2DM病程≥10年、血清ET水平≥100 ng/L、血浆MMP-9≥250ng/ml为影响其出现微血管病变的独立危险因素。Objective To explore correlation between endothelin(ET),matrix metalloproteinase-9(MMP-9)and microangiopathy in elderly patients with type 2 diabetes(T2DM),and analyze the risk factors of microangiopathy.Methods 153 T2DM patients who were treated in the hospital from August 2017 to March 2019 were enrolled.According to presence or absence of microangiopathy,they were divided into microangiopathy group(n=85)and non-microangiopathy group(n=68).The levels of ET and MMP-9 were compared between the two groups.The correlation between ET and MMP-9 was analyzed.The multivariate logistic regression analysis was applied to analyze risk factors affecting occurrence of microangiopathy.Results The levels of serum ET and MMP-9 in microangiopathy group were higher than those in non-microangiopathy group(P<0.05).The level of ET was positively correlated with level of MMP-9(r=0.469,P=0.000).Multiple logistic regression analysis showed that hypertension,heart disease,stroke,T2DM course not shorter than 10 years,level of serum ET not lower than 100ng/L and plasma MMP-9 not lower than 250ng/ml were risk factors affecting microangiopathy in T2DM patients(P<0.05).Conclusion The levels of ET and MMP-9 are correlated with occurrence of microangiopathy in T2DM patients.Patients with hypertension,heart disease,stroke,T2DM course≥10 years,serum ET level 100ng/L、plasma 250ng/ml are independent risk factors affecting occurrence of microangiopathy.
