摘  要:目的探讨磁共振血管成像用于诊断颅内动脉狭窄的临床价值。方法回顾性分析该院2017年5月-2018年5月收治的174例缺血性脑血管病患者的临床资料,所有患者均行经颅多普勒超声(对照组)与磁共振血管成像(观察组)检查;以数字减影血管造影(DSA)作为诊断金标准。比较两组特异性、敏感性、假阳性率、假阴性率以及检出率。结果①观察组诊断特异性、敏感性、假阳性率以及假阴性率分别为98.5%、84.5%、1.3%、3.6%;对照组分别为93.9%、83.0%、5.0%以及4.5%;两组比较差异无统计学意义(χ^2=0.852、0.741、0.936、0.879,P>0.05);②观察组检出颅内动脉狭窄296根,对照组检出256根,DAS检出312根;观察组检出率(94.87%)高于对照组(82.05%),但差异无统计学意义(χ^2=1.014,P>0.05)。结论磁共振血管成像能够有效反映脑血流动力学的变化情况,可作为临床中诊断颅内动脉狭窄的一线方案,值得推广应用。Objective To investigate the clinical value of magnetic resonance angiography in the diagnosis of intracranial artery stenosis.Methods The clinical data of 174 patients with ischemic cerebrovascular disease admitted from May 2017 to May 2018 in the hospital were retrospectively analysised.All patients underwent transcranial Doppler ultrasound(control group)and magnetic resonance angiography(observation group)examination;digital subtraction angiography(DSA)was used as the diagnostic gold standard.The two groups were compared for specificity,sensitivity,false positive rate,false negative rate,and detection rate.Results 1.The diagnostic specificity,sensitivity,false positive rate and false negative rate of the observation group were 98.5%,84.5%,1.3%,and 3.6%,respectively;the control group was 93.9%,83.0%,5.0%,and 4.5%,respectively.There was no statistically significant difference between the two groups(χ^2=0.852,0.741,0.936,0.879,P>0.05);2.296 roots of intracranial artery stenosis were detected in the observation group,256 were detected in the control group,and 312 were detected by DAS.The detection rate of the observation group(94.87%)was higher than that of the control group(82.05%),but the difference was not statistically significant(χ^2=1.014,P>0.05).Conclusion Magnetic resonance angiography can effectively reflect the changes of cerebral hemodynamics,and can be used as a first-line solution for the diagnosis of intracranial artery stenosis in clinical practice.It is worthy of popularization and application.
