摘　　要：目的讨论研究Liss/coombs卡应用于ABO疑难血型检测中的临床价值与意义。方法选择该院2018年1-6月7例ABO疑难血型患者作为研究对象。将患者1%红细胞悬液50μL与50μL的抗-A、抗-B标准血清在血型鉴定微凝胶柱卡上进行反应,之后将患者血清50μL与1%ABO试剂细胞A、B细胞液50μL进行反应并将反应液在Liss/coombs卡上进行检测并对不同的稀释浓度标准血清反应进行检测。结果抗-A,抗-B标准血清进行连续倍比稀释,分别以盐水法、柱凝集法及混合实验法测定各自最大稀释度的有效反应,盐水发最低,滴度仅有128;柱凝集法灵敏度达到512;而混合法测定结果较其余两种更高,需达到4096。结论盐水结合柱凝集法对ABO疑难血型进行检测具有灵敏度及准确率高的特点,尤其对正反定型不一致的疑难血型检测效果较好,值得临床广泛推荐应用。Objective To discuss the clinical value and significance of saline conjugate column in ABO difficult blood group detection.Methods Seven ABO patients with difficult blood group in the hospital from January to June 2018 were selected as the study subjects.One percent ofpatient’s erythrocytesuspension 50μL was reacted with 50μL of anti-A and anti-B standard serum,then 50μL of patient’s serum was reacted with 50μL one percentof ABO reagent cell A and B,and the reaction liquid was detected on Liss/coombs card and the standard serum reaction with different dilution concentration was detected.Results The standard serum of anti-A and anti-B was diluted continuously by multiple dilution.The maximum dilution of each standard serum was determined by saline solution method,column agglutination method and mixed experiment respectively.The saline hair was the lowest,and the titer was only 128.The sensitivity of column agglutination method was 512,and the results of mixed method were higher than those of the other two methods,requiring 4096.Conclusion Saline combined with column agglutination method has the characteristics of high sensitivity and accuracy in detecting ABO difficult blood group,especially for those with inconsistent forward andbackward typing,which is worthy of wide clinical recommendation and application.
