摘  要:目的探讨微创手术治疗脊柱骨质疏松压缩性骨折的疗效。方法方便择取该院骨科于2017年5月-2018年9年期间收治的84例脊柱骨质疏松压缩性骨折患者为研究对象,根据数字表法进行随机分组,包括对照组42例和观察组42例,对照组患者开展传统开放手术治疗,观察组患者开展微创手术治疗,比较两组患者临床疗效、围术期指标以及并发症情况。结果观察组患者临床疗效97.62%高于对照组80.95%,差异有统计学意义(χ^2=6.098,P<0.05);观察组患者手术时间(115.12±3.14)min、出血量(133.65±10.28)mL、下床时间(33.51±2.42)h、骨折愈合时间(14.12±1.25)周少于对照组(138.41±3.26)min、(216.37±12.72)mL、(52.43±3.65)h、(18.65±2.38)周,疼痛评分(2.25±0.35)分低于对照组(4.12±0.22)分,椎体高度(27.13±1.68)mm高于对照组(22.15±1.33)mm,差异有统计学意义(t=33.347、32.779、27.998、10.921、29.315、15.062,P<0.05);观察组患者并发症发生率7.14%低于对照组28.57%,差异有统计学意义(χ^2=6.574,P<0.05)。结论微创手术对脊柱骨质疏松压缩性骨折治疗效果相比传统手术更理想,能够缩短其骨折愈合时间,提升手术安全性,因此该种治疗方法可在临床中进行大力推广。Objective To investigate the efficacy of minimally invasive surgery for the treatment of osteoporotic compression fractures of the spine.Methods Convenient selection of eighty-four patients with osteoporotic compression fractures of the spine admitted to the hospital during the period from May 2017 to September 2018 were enrolled in the hospital.They were randomly assigned according to the digital table method,including 42 patients in the control group.Forty-two patients in the observation group were treated with conventional open surgery.The patients in the observation group underwent minimally invasive surgery.The clinical efficacy,perioperative index and complications were compared between the two groups.Results The clinical efficacy of the observation group was 97.62%higher than that of the control group(80.95%),and the difference was statistically significant(χ^2=6.098,P<0.05).The operation time of the observation group was(115.12±3.14)min,the amount of bleeding(133.65±10.28)mL,The time of bed-out(33.51±2.42)h and the time of fracture healing(14.12±1.25)weeks were less than that of the control group(138.41±3.26)min,(216.37±12.72)mL,(52.43±3.65)h,(18.65±2.38)weeks.The pain score(2.25±0.35)points was lower than that of the control group(4.12±0.22)points,and the height of the vertebral body(27.13±1.68)mm was higher than that of the control group(22.15±1.33)mm.The difference was statistically significant(t=33.347,32.779,27.998,10.921,29.315,15.062,P<0.05);the incidence of complications in the observation group was 7.14%lower than that in the control group(28.57%),and the difference was statistically significant(χ^2=6.574,P<0.05).Conclusion Minimally invasive surgery is more ideal for the treatment of osteoporotic compression fractures of the spine than conventional surgery.It can shorten the time of fracture healing and improve the safety of surgery.Therefore,this kind of treatment can be promoted in clinical practice.
