Pentacam引导下白内障手术切口对角膜散光的影响

摘　　要：目的探讨Pentacam引导下白内障手术切口对角膜散光的影响。方法方便选取该院2017年3-9月接诊的白内障患者100例,将其按照入院时间的不同分成研究组50例和对照组50例。对照组患者行150°方位透明角膜切口白内障超声乳化联合人工晶体植入术;研究组在Pentacam引导下行最大散光轴位透明角膜切口白内障超声乳化联合人工晶体植入术。对比两组患者术前术后的角膜散光和裸眼视力情况。结果研究组患者散光范围1~2 D术前(1.65±0.27)、>2 D(2.95±0.56),术后1~2 D(0.87±0.23)、>2 D(1.45±0.36);对照组散光范围1~2 D术前(1.68±0.22)、>2 D(2.98±0.56),术后1~2 D(1.57±0.29)、>2 D(2.75±0.76),可见术前两组患者的角膜散光对比差异无统计学意义(t=0.609、0.268,P>0.05),术后研究组的散光明显低于对照组(t=13.373、10.931,P<0.05);研究组术前裸眼视力1~2 D(0.16±0.17)、>2 D(0.23±0.15),术后1~2 D(0.89±0.25)、>2 D(0.81±0.23);对照组患者术前裸眼视力1~2 D(0.15±0.16)、>2 D(0.19±0.15),术后1~2 D(0.65±0.24)、>2 D(0.70±0.21),可见术前两组患者差异无统计学意义(t=0.303、1.333,P>0.05),研究组患者术后裸眼视力要优于对照组(t=8.896、8.324,P<0.05)。结论在Pentacam引导进行白内障手术,选择合适的切口对于术后散光的矫正有重要意义。Objective To investigate the effect of Pentacam-guided cataract incision on corneal astigmatism.Methods Convenient selection of one hundred patients with cataract who were admitted to our hospital from March 2017 to September2017 of the same year were enrolled.According to the time of admission,50 patients were divided into study group and 50 patients in the control group.The control group underwent 150 degree azimuth clear corneal incision phacoemulsification combined with intraocular lens implantation.The study group under the Pentacam guided maximum astigmatism axial clear corneal incision phacoemulsification combined with intraocular lens implantation.Corneal astigmatism and naked eye vision were compared between the two groups before and after surgery.Results The astigmatism range of the study group was 1~2 D before operation(1.65±0.27),>2 D(2.95±0.56),postoperative 1~2 D(0.87±0.23),>2 D(1.45±0.36);control group astigmatism range 1~2 D preoperative(1.68±0.22),>2 D(2.98±0.56),postoperative 1~2 D(1.57±0.29),>2 D(2.75±0.76),no difference in corneal astigmatism between the two groups before surgery(t=0.609,0.268,P>0.05),the astigmatism of the postoperative study group was significantly lower than that of the control group(t=13.373,10.931,P<0.05).The uncorrected visual acuity of the study group was 1~2 D(0.16±0.17),>2 D(0.23±0.15),and after operation 1~2 D(0.89±0.25),>2 D(0.81±0.23);the uncorrected visual acuity of the control group was 1~2 D(0.15±0.16),>2 D(0.19±0.15),and after operation 1~2 D(0.65±0.24),>2 D(0.70±0.21),there was no difference between the two groups before operation(t=0.303、1.333,P>0.05).The visual acuity of the patients in the study group was better than that of the control group(t=8.896、8.324,P<0.05).Conclusion In the cataract surgery guided by Pentacam,the selection of a suitable incision is important for the correction of postoperative astigmatism.
