消化性溃疡的中医证型及证素研究

摘　　要：目的:探讨和归纳消化性溃疡的中医证型及证素分布规律。方法:从知网、维普、万方等文献数据库筛选收集2007年1月至2019年5月有关消化性溃疡辨证的文献材料,进行证型收集总结,再分解、提取其证素,建立数据库,运用SPSS 21.0对其证型、证素分布进行频次分析,探讨其规律,最后运用中医辨证理论对研究结果进行综合分析。结果:①检索到相关文献1569篇,依据文献筛选原则最终确定文献为37篇,病例数合计6300例,收入证型共20个,病位证素5个,病性证素10个。②肝胃不和是消化性溃疡最常见的证型,其次是脾胃湿热、脾胃虚弱等。③病位证素主要是胃、脾、肝,涉及胃络和心。④病性证素主要为气滞、热、湿等。结论:肝胃不和证是消化性溃疡患者的主要证型;胃、脾、肝是消化性溃疡的关键病位证素;气滞、热、湿是消化性溃疡的关键病性证素;消化性溃疡的病性特点是虚实夹杂、以实为主。Objective:To explore and summarize the TCM syndrome type and syndrome element distribution of peptic ulcer.Methods:The literature materials related to the syndrome differentiation of peptic ulcer in the past 10 years had been screened and collected from the literature databases of Zhiwang,Weipu,Wanfang,etc.to collect and summarize the syndrome types,then decomposed and extracted the syndrome elements,established the database.Used SPSS21.0 to analyze the syndrome types and the distribution of syndrome elements,explored the law,and finally used the theory of TCM syndrome differentiation to analyze the research results comprehensively.Results:①A total of 1569 related literatures were retrieved.According to the principle of literature screening,37 literatures were finally determined.The total number of cases was 6300.There were 20 income syndrome types,5 syndrome elements of disease location,and 10 syndrome elements of disease nature.②The syndrome of incoordination between liver and stomach was the most common syndrome type of peptic ulcer,followed by damp-heat of spleen and stomach and weakness of spleen and stomach.③The syndrome elements of disease location were mainly stomach,spleen and liver,involving stomach collaterals and heart.④The main syndrome elements of disease nature were qi stagnation,heat and dampness.Conclusion:The syndrome of incoordination between liver and stomach is the main syndrome type of peptic ulcer;stomach,spleen and liver are the key syndrome elements of peptic ulcer;qi stagnation,heat and dampness are the key syndrome elements of peptic ulcer;the disease characteristics of peptic ulcer are the mixture of deficiency and excess,and the main syndrome is the excess.
