摘 要:目的分析基质金属蛋白酶(MMPs)与子痫前期发病的相关性。方法随机选取2016年2月~2018年2月河北省唐山市妇幼保健院子痫前期患者51例为研究对象,根据其病情严重程度将其分为重度组(n=22)和轻度组(n=29);将同期正常足月妊娠妇女纳入对照组(n=30)。比较三组血清MMP-2、MMP-9水平,分析血清MMP-2、MMP-9水平与子痫前期发病的相关性,评价MMP-2、MMP-9水平对重度子痫前期的预测价值。结果重度组血清MMP-2、MMP-9水平低于轻度组和对照组,轻度组血清MMP-2、MMP-9水平低于对照组(均P <0.05)。Spearman相关分析显示,血清MMP-2、MMP-9水平与子痫前期发病及病情严重程度呈负相关(r=-0.839、-0.747,P <0.001)。血清MMP-2水平预测重度子痫前期的临界值为54.89 pg/mL,敏感度、特异性分别为0.818、0.828;血清MMP-9水平预测重度子痫前期的临界值为175.62 pg/mL,敏感度、特异性分别为0.818、0.897。结论血清MMP-2、MMP-9水平与子痫前期发病及病情严重程度密切相关,对重度子痫前期有一定预测价值。Objective To analyze the correlation between matrix metalloproteinases(MMPs) and onset of preeclampsia.Methods From February 2016 to February 2018, 51 cases with preeclampsia patients in Tangshan Maternal and Child Health Hospital of Hebei Province were randomly selected for the study. According to the severity of disease, they were divided into severe group(n = 22) and mild group(n = 29). Women with normal term pregnancy at the same time were included in control group(n = 30). The levels of serum MMP-2 and MMP-9 were compared among three groups. The correlation between serum levels of MMP-2, MMP-9 and onset of preeclampsia was analyzed. The predictive value of MMP-2 and MMP-9 levels on severe preeclampsia was evaluated. Results The levels of serum MMP-2 and MMP-9 in severe group were lower than those in mild group and control group, and the levels serum MMP-2 and MMP-9 in mild group were lower than those in control group(all P < 0.05). Spearman correlation analysis showed that serum MMP-2 and MMP-9 levels were negatively correlated with onset of preeclampsia and disease severity(r =-0.839,-0.747;P <0.001). The cut-off value of serum MMP-2 level was 54.89 pg/mL in predicting severe preeclampsia, and the sensitivity, specificity were 0.818, 0.828. The cut-off value of serum MMP-9 level was 175.62 pg/mL in predicting severe preeclampsia, and the sensitivity, specificity were 0.818, 0.897. Conclusion The levels of serum MMP-2 and MMP-9 are closely related to the onset of preeclampsia and severity of preeclampsia, which has certain predictive value for severe preeclampsia.
