摘 要:目的:探讨参麦注射液治疗急性缺血性脑卒中早期神经功能恶化的疗效及对患者血清细胞因子水平的影响。方法:选取2017年5月至2019年5月唐山市协和医院收治的急性缺血性脑卒中早期神经功能恶化患者118例,按随机数字表法分为对照组和观察组,每组59例。两组患者均给予常规西药治疗,观察组患者在此基础上加用参麦注射液静脉滴注,两组患者的疗程均为4周。观察两组患者的临床疗效、不良反应发生情况,治疗前后美国国立卫生院卒中神经功能缺损评分量表(national institutes of health stroke scale,NIHSS)评分、日常生活能力(activities of daily living,ADL)评分及血清细胞因子[肿瘤坏死因子α(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素6(IL-6)、白细胞介素18(IL-18)及脑源性神经营养因子(BDNF)]水平变化情况。结果:治疗后,观察组患者的NIHSS评分明显低于治疗前且明显低于对照组治疗后,ADL评分明显高于治疗前且明显高于对照组治疗后,差异均有统计学意义(P<0.05)。治疗后,观察组患者TNF-α、hs-CRP、IL-6及IL-18水平明显低于治疗前且明显低于对照组治疗后,BDNF水平明显高于治疗前且明显高于对照组治疗后,差异均有统计学意义(P<0.05)。观察组患者的总有效率为88.14%(52/59),明显高于对照组的77.97%(46/59),差异有统计学意义(P<0.05)。治疗期间,观察组、对照组患者不良反应发生率分别为15.25%(9/59)、27.12%(16/59),差异无统计学意义(P>0.05)。结论:参麦注射液治疗急性缺血性脑卒中早期神经功能恶化的效果佳,可促进患者神经功能的恢复,改善血清细胞因子水平,抑制炎症反应。OBJECTIVE:To probe into the efficacy of Shenmai injection in the treatment of neurological deterioration in early stage of acute ischemic stroke and its effects on serum cytokines.METHODS:Totally 118 patients with neurological deterioration in early stage of acute ischemic stroke admitted into Tangshan Union Medical College Hospital from May 2017 to May 2019 were extracted to be divided into the control group and the observation group via the random number table,with 59 cases in each group.Both groups were given conventional western medicine treatment,on this basis,the observation group was treated with intravenous infusion of Shenmai injection.The course of treatment of both groups were 4 weeks.Clinical efficacy,incidence of adverse drug reactions of two groups were observed.National institutes of health stroke scale(NIHSS),activities of daily living(ADL)and serum cytokines[tumor necrosis factorα(TNF-α),high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),interleukin-18(IL-18)and brain-derived neurotrophic factor(BDNF)]of two groups were compared before and after treatment.RESULTS:After treatment,NIHSS score in the observation group was significantly lower than that before treatment and the control group after treatment,and ADL score was significantly higher than that before treatment and the control group after treatment,with statistically significant differences(P<0.05).After treatment,the levels of TNF-α,hs-CRP,IL-6 and IL-18 in the observation group were significantly lower than those before treatment and the control group after treatment,and the levels of BDNF were significantly higher than those before treatment and the control group after treatment,the differences were statistically significant(P<0.05).The total effective rate of the observation group was 88.14%(52/59),significantly higher than that of the control group[77.97%(46/59)],with statistically significant difference(P<0.05).During the treatment,the incidence of adverse drug reactions in the observation group and the control group w
