新医改形势下医院药学服务模式的转变对医院医保药品费用支出的影响

摘　　要：目的:探索在新医改政策下医院药学部门的发展路径,为医院开展药学服务变革提供参考。方法:调取2015年4月至2019年3月北京市大兴区人民医院(以下简称“我院”)药品使用数据,通过比较实施改革前后我院药占比、门诊和住院次均药品费用及处方合理率等情况,评价我院药剂科在改革中发挥的作用。结果:我院药剂科通过多措并举,在提高合理用药水平、降低患者用药负担方面取得了一定成效,我院药占比由实施改革前的约50%降至实施改革后的约30%;门诊次均药品费用由实施改革前的184元降至实施改革后的147元,降低了20.11%;住院次均药品费用由实施改革前的4600元降至实施改革后的3489元,降低了24.15%。结论:我院药学服务转型激发了药师工作积极性,实现了药剂科职能转变和价值提升,有效降低了医院医保药品费用支出。OBJECTIVE:To explore the development path of pharmacy department under the new medical reform policy,so as to provide reference for the reform of pharmaceutical services.METHODS:Data of drug application from Apr.2015 to Mar.2019 in People’s Hospital of Beijing Daxing District(hereinafter referred to as“our hospital”)were retrieved,the role of pharmacy department in medical reform was evaluated by comparing the medicine proportion,average outpatient and inpatient drug costs and rational rate of prescriptions before and after the reform.RESULTS:Through multiple measures,the pharmacy department of our hospital had achieved a certain results in improving rational drug application and reducing medical burden of patients.The medicine proportion in our hospital had decreased from about 50%of before reform to about 30%of after reform;the average drug cost in outpatient had decreased from 184 yuan of before reform to 147 yuan of after reform,with a decrease rate of 20.11%;the average drug cost of inpatient had decreased from 4600 yuan of before reform to 3489 yuan of after reform,with a decrease rate of 24.15%.CONCLUSIONS:The transformation of pharmaceutical service in our hospital has stimulated the working enthusiasm of pharmacists,realized the functional transformation and value enhancement of the pharmacy department,which has effectively reduced the expenses of medical insurance drugs for our hospital.
