炙甘草汤加味联合冻干重组人脑利钠肽治疗慢性心力衰竭68例

摘　　要：目的:观察炙甘草汤加味联合冻干重组人脑利钠肽治疗慢性心力衰竭的临床疗效。方法:将136例慢性心力衰竭患者按照随机数字表法分为对照组和观察组,每组各68例。对照组予以冻干重组人脑利钠肽治疗,观察组在对照组的基础上联合炙甘草汤加味治疗。比较两组患者治疗前后左心室功能[左心室舒张末期内径(left ventricle end diastolic diameter,LVEDD)、左心室收缩末期内径(left ventricular end systolic diameter,LVESD)及左心室射血分数(left ventricular ejection fraction,LVEF)]、血清基质金属蛋白酶抑制剂-1(tissue inhibitor of metalloproteinase-1,TIMP-1)、人心型脂肪酸结合蛋白(human heart type fatty acid binding protein,H-FABP)水平、24 h平均心率、6分钟步行距离以及健康调查简表(short from health survey,SF-36)评分的变化情况。结果:两组患者治疗后LVEDD、LVESD低于治疗前,LVEF高于治疗前,且观察组LVEDD、LVESD低于对照组治疗后,LVEF高于对照组治疗后,差异均有统计学意义(P<0.05)。两组患者治疗后H-FABP水平低于治疗前,TIMP-1水平高于治疗前,且观察组H-FABP水平低于对照组治疗后,TIMP-1水平高于对照组治疗后,差异均有统计学意义(P<0.05)。两组患者治疗后24 h平均心率、6分钟步行距离大于治疗前,且观察组大于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后SF-36评分高于治疗前,且观察组高于对照组,差异具有统计学意义(P<0.05)。结论:炙甘草汤加味联合冻干重组人脑利钠肽治疗慢性心力衰竭,可有效改善患者左心室功能,调节血清TIMP-1、H-FABP表达,增加24 h平均心率以及6分钟步行距离,提高患者生活质量。Objective:To observe the clinical curative effect of Supplemented Honey-Fried Licorice Decoction in combination with lyophilized recombinant human brain natriuretic peptide on chronic heart failure.Methods:A total of 136 patients with chronic heart failure were divided into the control group and the observation group according to the random number table method,with 68 cases in each group.The control group was treated with lyophilized recombinant human brain natriuretic peptide,and the observation group was treated additionally with Supplemented Honey-Fried Licorice Decoction on the basis of the control group.The changes of left ventricular function including left ventricular end diastolic diameter(LVEDd),left ventricular end systolic diameter(LVESD) and left ventricular ejection fraction(LVEF),tissue inhibitor of metalloproteinase-1(TIMP-1),human heart type fatty acid binding protein(H-FABP)level,24-h mean heart rate,6-minute walking distance and the MOS item short from health survey(SF-36) before and after the treatment in the two groups were compared.Results:After the treatment,LVEDD and LVESD of the two groups were lower than those before treatment,while LVEF was higher than that before the treatment,and LVEDD and LVESD of the observation group were lower than those of the control group,LVEF was higher than that of the control group,and all the differences were statistically significant(P<0.05).The level of H-FABP in either of the two groups was lower than that before the treatment,and the level of H-FABP in the observation group was lower than that in the control group,while the level of TIMP-1 in the observation group was higher than that in the control group,and all the differences were statistically significant(P<0.05).The 24-hour mean heart rate and 6-minute walking distance of the two groups were higher than those before the treatment,and the observation group′s were larger than those in the control group,and all the difference were statistically significant(P<0.05).The SF-36 score of the two groups wa
