摘 要:目的:了解珠海市某三甲抗菌药物管理措施及效果评估。方法:利用医院HⅠS系统收集医院2011年1月至6月,2012年至2018年抗菌药物数据。结果:门诊抗菌药物处方比例、门诊小儿上呼吸道感染抗菌药物使用率、门诊急性支气管炎抗菌药物使用率、住院患者抗菌药物使用率、这些指标均低于整治前(P<0.05)。此外,Ⅰ类切口手术围手术期抗菌药物预防用药率、Ⅰ类切口手术围手术期术前0.5~1h抗菌药物预防用药比例、Ⅰ类切口手术抗菌药物疗程控制在24h内比例较整治前明显改善(P<0.05),门诊、住院抗菌药物费用/药品总费用的比例整治前后无明显变化。结论:开展抗菌药物专项整治后,我院抗菌药物使用日益合理,对于一些不合理的方面仍需探索新的管理措施促进抗菌药物合理使用,遏制细菌耐药。Objective: Retrospective analysis of antimicrobial management measures and continuous improvement in a grade iii class a hospital in zhuhai. Methods: Collected and analyzed antibacterial drug data from the hospital’s HIS system from January to June 2011 and from 2012 to 2018. Results: The ratio of antimicrobial agents prescription in outpatient service, the utilization rate of antimicrobial agents for pediatric upper respiratory tract infection in outpatient service, the utilization rate of antimicrobial agents for acute bronchitis in outpatient service, and the utilization rate of antimicrobial agents in inpatient service were all lower than those before retreatment(P<0.05). In addition, the rate of antibacterials prophylaxis in perioperative period of class I incision surgery and the rate of perioperative period of class I incision surgery were 0. The ratio of prophylactic use of antimicrobial agents within 5-1 h and the ratio of antibiotic course control in class I incision surgery within 24 h were significantly improved compared with that before treatment(P<0.05). There was no significant change in the ratio of outpatient and inpatient antimicrobial drug expense/total drug expense before and after treatment. Conclusion: the application of antimicrobial agents is becoming more and more reasonable, but new management measures should be explored to promote rational use of antimicrobial agents and curb bacterial resistance.
