摘  要:目的:观察电针结合回医烙灸对脊髓损伤后神经源性膀胱大鼠逼尿肌细胞凋亡的影响。方法:将SD大鼠40只随机分为电针结合回医烙灸组(电+烙组)、电针组、模型对照组、正常组每组10只。采用重物坠落法建立脊髓损伤后神经源性膀胱大鼠模型。电+烙组在电针次髎治疗基础上,烙灸关元治疗;电针组仅采用电针次髎方法;模型组仅造模,不治疗;正常组不采用任何干预措施。各组分别在第10次治疗结束24 h后取标本,常规石蜡包埋切片,TUNEL法检测细胞凋亡,免疫组化SP法检测BcL-2、BaxA表达。结果:电+烙组、电针组与模型对照组比较,膀胱逼尿肌TUNEL检测细胞阳性率显著降低(P<0.001);膀胱逼尿肌中BcL-2蛋白表达显著增高(P<0.01);膀胱逼尿肌中BaxA蛋白表达显著降低(P<0.01)。结论:电针次髎、烙灸关元可改善脊髓损伤后神经源性膀胱大鼠膀胱功能,其作用机制可能与膀胱逼尿肌中Bcl-2升高、BaxA蛋白表达下调,逼尿肌细胞凋亡减少有关。Objective:To observe the effect of electro-acupuncture and Hui medicine flatiron moxibustion on detrusor cell apoptosis of rat with spinal cord injury-induced neurogenic bladder.Methods:40 SD rats were randomly divided into electro-acupuncture combined with Hui medicine flatiron moxibustion group(electroflatiron group),electro-acupuncture group,model control group and normal group,10 rats in each group.The rat model of spinal cord injury-induced neurogenic bladder was established by weight-drop.Electro-flatiron group was treated with flatiron moxibustion on Guanyuan on the basis of electro-acupuncture at Ciliao.After 24 hours of ending the 10th treatment,respectively in each group,the specimens were taken out;the sections were embedded with conventional paraffin wax;the cell apoptosis was tested by TUNEL;Bcl-2 and BaxA protein expressions were tested by immunohistochemistry SP.Results:Compared electro-flatiron group and electro-acupuncture group with model control group,the positive rate of by TUNEL-tested bladder detrusor cells was significantly decreased(P﹤0.001);in bladder detrusor,Bcl-2 protein expression was significantly increased(P﹤0.01),while BaxA protein expression was significantly decreased(P﹤0.01).Electro-acupuncture at Ciliao and flatiron moxibustion on Guanyuan could improve the bladder function of rat with spinal cord injury-induced neurogenic bladder,and their mechanism might be related to the increase of Bcl-2 protein expression and the decrease of BaxA protein expression in bladder detrusor as well as to the reduction of detrusor cell apoptosis.Conclusion:Electro-acupuncture combined with Hui medicine flatiron moxibustion can significantly improve the neurogenic bladder function of model rat with acute spinal cord injury,and reduce the detrusor cell apoptosis,which may be related to regulating the increase of Bcl-2 protein expression and the decrease of BaxA protein expression in bladder detrusor of spinal cord injury.
