摘　　要：目的了解某幼儿园一起流感暴发疫情的流行特征,探讨暴发原因,为其防控提供科学依据。方法现场进行流行病学调查并采样检测,对不同人群的罹患率进行卡方检验。结果此次疫情共持续7日,累计发病21例,教师罹患率为20.59%、学生罹患率为3.90%,人群罹患率差异有统计学意义(χ^2=17.10,P<0.01);病例主要症状为咳嗽(95.24%)、发热(85.71%);4例病例咽拭子标本经检测均为甲型H1N1阳性;全园师生近一年来均无流感疫苗接种史;教师病例中仅2例具有就诊史。结论根据流行病学调查、临床症状及实验室检测结果,综合判断此次疫情为一起甲型H1N1流感病毒引起的暴发疫情。教师在幼儿园流感疫情蔓延中的作用不容忽视,应加强对患病教师的隔离治疗。Objective To get an insight into the epidemic characteristics of an outbreak of influenza in a kindergarten,explore the causes of the outbreak,and to provide a scientific basis for its prevention and control.Methods On-site epidemiological investigation and sampling detection were conducted,and the incidence rates of different groups were tested by chi-square test.Results The epidemic lasted for 7 days,with a cumulative incidence of 21 cases in total.The incidence rates of teachers and students were 20.59%and 3.90%,respectively.The incidence rate of the population was statistically significant(χ^2=17.10,P<0.01).The main symptoms of the cases were cough(95.24%)and fever(85.71%).The pharyngeal swab specimens from 4 cases were tested positive for influenza A(H1N1)virus.There has been no history of influenza vaccination among teachers and students in the whole kindergarten in the past year.Only two of the teacher cases had a history of treatment.Conclusion According to the epidemiological investigation,clinical symptoms and laboratory test results,it is comprehensively determined that this outbreak is caused by A(H1N1)influenza virus.The role of teachers in the spread of influenza in kindergartens could not be ignored.The isolation treatment of sick teachers should be strengthened.
