摘  要:目的:探讨血细胞分析相关指标在登革热病毒感染中的辅助诊断价值。方法:分析140例进行登革热病毒核酸检测发热患者,阳性组(DENV)与阴性组(NC)患者血常规检测项目中的白细胞(WBC)总数,中性粒细胞(Neu)值,淋巴细胞(Lym)值,血小板(PLT)总数,计算中性粒细胞绝对值与淋巴细胞绝对值比值(NLR),血小板总数与淋巴细胞绝对值比值(PLR)等数据,采用受试者工作特征曲线(ROC曲线)评价以上指标对登革热的预测价值。结果:阳性组与阴性组的WBC、Neu、Lym、PLT、NLR、PLR等指标组间均存在显著性差异(P<0.05),DENV组的值均不同程度低于NC组。ROC曲线下面积(AUROC)提示Plt诊断效能最高,截断值为89.50×109L-1,灵敏度为0.985,特异性为0.676,约登指数为1.661。联合Plt、WBC和Neu三项进行平行实验,阳性预测值为85.54%,阴性预测值为94.74%,诊断符合率达89.29%。建立新登革因子诊断模型(NDF),NDF=5.923-0.0378Plt-0.425×WBC-0.01×PLR,诊断效能较单个指标有所提高。结论:新登革因子对登革热具有较高的预测价值。Objective To investigate the value of predicting dengue virus infection using hematology analyzer multi-parameters.Methods 140 cases which were performed DENV-RNA test were analyzed retrospectively,140 cases were divided into DENV group and NC group.WBC,neutrophil count(Neu),lymphocyte count(Lym),plateletcount(Plt),neutrophil-lymphocyte ratio(NLR),platelet-lymphocyte ratio(PLR)were evaluatedfor predicting dengue fever using ROC curve.Results WBC,Neu,Lym,Plt,NLR,PLR in DENV group were significant lower than NC group,P<0.05.Plthadmost diagnostic efficiency,cut-off value was 89.50×109 L-1,sensitivity was 0.985,specificity was 0.676,youden index was 1.661.Plt,WBC,and Neu were combined in parallel test,positive predictive value was 85.54%,negative predictive value was 94.74%,diagnose accordance rate was 89.29%.In this study,new dengue factor named NDF was set up,NDF=5.923-0.0378Plt-0.425×WBC-0.01×PLR,NDF diagnostic efficacy was higher than individual indicators.Conclusion New dengue factor had great value for predicting dengue fever.
