摘 要:目的探讨超声剪切波弹性成像技术在糖尿病周围神经病变(DPN)的诊断效能中的临床应用价值。方法选取2018年7月~2019年7月重庆市涪陵中心医院收治的176例2型糖尿病(T2DM)患者,其中合并DPN者88例,未合并DPN者(NDPN者)88例。所有患者均进行神经肌电图、高频超声及超声剪切波弹性成像检查,比较三者诊断DPN的敏感度、特异度及曲线下面积。结果神经肌电图的敏感度为0.951,特异度为0.857,曲线下面积为0.709(95%CI 0.654~0.764);高频超声的敏感度为0.927,特异度为0.846,曲线下面积为0.685(95%CI 0.607~0.763);剪切波弹性成像的敏感度为0.994,特异度为0.897,曲线下面积为0.881(95%CI 0.804~0.958)。结论剪切波弹性成像在糖尿病周围神经病变的诊断中具有一定的应用价值。Objective To analyze the diagnostic effect of shear wave elastrography in diabetic peripheral neuropathy(DPN).Methods From July 2018 to July 2019,176 T2 DM patients were chosen as study objects,including 88 DPN patients and 88 non-diabetic peripheral neuropathy(NDPN)patients.All patients were examined by neuroelectromyography,high frequency ultrasound and shear wave elastography,and the sensitivity,specificity and area under curve were compared.Results The sensitivity of neuroelectromyography was 0.951,the specificity was 0.857,the area under the curve was 0.709(95%CI 0.654~0.764);the sensitivity of high frequency ultrasound was 0.927,the specificity was 0.846,the area under the curve was 0.685(95%CI 0.607~0.763);the sensitivity of shear wave elastic imaging was 0.994,the specificity was 0.897,the area under the curve was 0.881(95%CI0.804~0.958).Conclusion Shear wave elastography has a certain value in the diagnosis of diabetic peripheral neuropathy.
