摘  要:目的:探讨乙型肝炎病毒表面抗原(HBsAg)阳性产妇的新生儿Th1/Th2细胞因子表达水平及相关性。方法:选取2018年1月~2019年1月我院收治的HBsAg阳性产妇126例及其新生儿126例,根据新生儿脐血的乙肝5项检查及HBVDNA检查结果,将23例检测结果阳性的母婴为宫内感染组,103例检测结果阴性的母婴为宫内未感染组,再选取同期50例健康母婴为对照组。比较产妇外周血、新生儿脐血TH1/TH2细胞因子γ-干扰素(IFN-γ)、白细胞介素-4(IL-4)、白细胞介素-6(IL-6)、白细胞介素-18(IL-18)水平。采用Pearson相关性分析细胞因子表达水平间相关性。结果:对照组、宫内未感染组、宫内感染组产妇外周血IFN-γ、IL-18水平逐渐降低,IL-4、IL-6水平逐渐增加,比较有明显差异(P<0.05);对照组、宫内未感染组、宫内感染组新生儿脐血IFN-γ、IL-4、IL-6、IL-18水平比较无明显差异(P>0.05);产妇外周血IFN-γ与IL-4呈负相关(r=-0.860,P<0.05),与IL-6呈负相关(r=-0.727,P<0.05),与IL-18呈正相关(r=-0.433,P<0.05),IL-4与IL-6呈正相关(r=0.766,P<0.05),与IL-18呈负相关(r=-0.537,P<0.05),IL-6与IL-18呈负相关(r=-0.428,P<0.05);新生儿脐血Th1/Th2细胞因子IFN-γ、IL-4、IL-6、IL-18间均无显著相关性(P>0.05)。结论:HbsAg阳性产妇Th1/Th2细胞系统功能紊乱,相关因子IFN-γ、IL-4、IL-6、IL-18表达水平异常,影响产妇体内HBV清除,易引起胎儿发生宫内感染。Objective:To investigate the Th1/Th2 cytokine expression and correlation in neonates with hepatitis B virus surface antigen(HBsAg)-positive mothers.Methods:A total of 126 HBsAg-positive women and 126 neonates admitted in our hospital from January 2018 to January 2019 were selected.Based on the results of five tests of hepatitis B and HBVDNA in neonatal umbilical blood,23 mothers and babies with positive test results were set as intrauterine infection group,and 103 mothers and babies with negative test results were set as intrauterine uninfected group.Then 50 healthy mothers and infants were selected as the control group.The levels of TH1/TH2 cytokines interferon-γ(IFN-γ),interleukin-4(IL-4),interleukin-6(IL-6),and interleukin-18(IL-18)in maternal peripheral blood and neonatal umbilical cord blood were compared.Pearson correlation was used to analyze the correlation between cytokine expression levels.Results:Among the control group,intrauterine uninfected group and intrauterine infectedgroup,levels of IFN-γand IL-18 in the peripheral blood of women had a decreasing trend,meanwhile,the levels of IL-4 and IL-6 showed an increasing trend(P<0.05).The levels of IFN-γ,IL-4,IL-6,and IL-18 in neonatal umbilical blood had no statistical difference among three groups(P>0.05).IFN-γin maternal peripheral blood was negatively correlated with IL-4(r=-0.860,P<0.05)and IL-6(r=-0.727,P<0.05),while positively correlated with IL-18(r=-0.433,P<0.05);IL-4 was positively correlated with IL-6(r=0.766,P<0.05),and negatively correlated with IL-18(r=-0.537,P<0.05).IL-6 was negatively correlated with IL-18(r=-0.428,P<0.05).Conclusion:The Th1/Th2 cell system of HBsAg positive pregnant women is dysfunctional,and the expression level of related factors IFN-γ,IL-4,IL-6 and IL-18 is abnormal,which may affect the elimination of HBV in pregnant women and cause intrauterine infection.
