摘  要:目的应用高级建模迭代重建(ADMIRE)算法进行颅脑CT低剂量扫描,观察其成像质量和辐射剂量。方法选择颅脑CT检查未见异常的受试者60例,随机分成常规剂量组和低剂量组各30例。常规剂量组设定管电压为120 kV,扫描后机器自动采用滤波反投影重建(FBP)算法进行重建,图像记为A组;低剂量组设定管电压为100 kV,其余扫描参数与常规剂量组相同,对扫描数据采用FBP算法进行重建,图像记为B组;手动添加ADMIRE算法重建低剂量组数据,将新图像记为C组。扫描后分别记录常规剂量组与低剂量组的剂量长度乘积(DLP)和CT剂量加权指数(CTDIvo1),计算辐射有效剂量(ED);用客观方法和主观方法评价A、B、C三组的图像质量。结果低剂量组的DLP、CTDIvol、ED均低于常规剂量组(P均<0.05)。客观图像质量评价显示,B组灰质信噪比、白质信噪比均低于A组和C组(P均<0.05),B组灰白质对比噪声比低于C组(P<0.05);主观图像质量评价显示,A、B、C三组图像≥3分者分别占96.7%、93.3%、96.7%,三组间比较差异无统计学意义(P均>0.05)。结论在颅脑CT检查中,应用ADMIRE算法可以在保证图像质量的前提下降低受试者的吸收辐射剂量。Objective To investigate the clinical application of low-dose scan combined with advanced modeled iterative reconstruction(ADMIRE)algorithm in the brain CT examination.Methods Sixty healthy subjects who received brain CT examination were randomly divided into the regular dosage group and low dosage group,with 30 cases in each group.Cases in the regular dosage group received 120 kV tube voltage and were reconstructed by traditional filtered back projection(FBP)and were labeled as group A.Cases in the low dosage group received 100 kV tube voltage and the data were reconstructed by traditional filtered back projection(FBP)and the images were labeled as group B.Cases receiving 100 kV tube voltage were calculated by ADMIRE,and the images were labeled as group C.The dose-length product(DLP)and volume-weighted CT dose index(CTDIvol)were recorded,then we calculated the effective dose(ED).The image quality of groups A,B,and C were evaluated by subjective and objective methods.Results DLP,CTDIvol,and ED in the low dosage group were significant lower than those of the regular dosage group,with statistical difference(all P<0.05).The objective image quality evaluation showed that the gray value signal-to-noise ratio(SNR)in the group B was significantly lower than those in groups A and C(both P<0.05);the white and gray matter contrast-to-noise ratio in the group B was significantly lower than that in the group C(P<0.05).The objective image quality evaluation showed that the subjects with images≥3 points in the groups A,B,and C accounted for 96.7%,93.3%,and 96.7%,respectively,with no significant difference between the three groups(all P>0.05),indicating that the evaluation system was consistent.Conclusion In the brain CT examination,the application of ADMIRE algorithm can significantly reduce radiation dose under the guarantee of the image quality.
