手术室血源性职业暴露现状调查与影响因素分析

摘　　要：目的调查分析手术室血源性职业暴露现状及其影响因素,并针对性拟定制度与流程的改进措施。方法选取2017年7月至2019年7月本院手术室的69名护理人员,收集基本信息,采用单因素和多因素logistic回归分析影响手术室血源性职业暴露的独立危险因素。结果69名护理人员中发生血源性职业暴露17名(24.64%)。其中手术缝合操作刺伤7名,手术操作器械割伤4名,清理医疗废物损伤3名,污染针头损伤2名,双手回套针帽1名。影响手术室血源性职业暴露的独立危险因素包括医务人员的工作年限、学历、血源性职业暴露认知情况(P<0.05)。结论针对血源性职业暴露的风险因素进行培训,规范操作流程,完善管理制度,加强防护,可预防或减少血源性职业暴露的发生。Objective To investigate and analyze the status and influencing factors of hematogenous occupational exposure in the operating room,and target to formulate system and the improvement measures of the process.Methods We selected 69 nursing staff in our operating room from July 2017 to July 2019,collected basic information,and univariate and multifactor logistic regression analysis were used to analyze the independent risk factors affecting hematogenous occupational exposure in the operating room.Results Among the 69 nursing staff,17(24.64%)had hematogenous occupational exposure.Among them,7 of which were stab wounds by surgical sutures,4 were cuts by surgical instruments,3 were injured by cleaning medical wastes,2 were injured by contaminated needles,and 1 was a needle cap that was put back with both hands.The independent risk factors that affect hematogenous occupational exposure in the operating room included medical staff's working years,education background,and cognition of hematogenous occupational exposure(P<0.05).Conclusion Targeting to train for the risk factors of hematogenous occupational exposure,standardizing the operation process,perfecting the management system,and strengthening protection,which can prevent or reduce the occurrence of hematogenous occupational exposure.
