摘　　要：目的探讨儿童神经母细胞瘤化疗初期血源性感染的相关因素,提醒护理人员应高度警觉,避免该不良事件的发生。方法选取2016年8月至2019年8月广州市某三级甲等专科医院儿童肿瘤科收治的神经母细胞瘤化疗患儿150例,其中发生血源性感染27例,收集所有患儿血常规、化疗组合及不良反应相关资料,采用单因素和多因素logistic回归分析导致神经母细胞瘤化疗患儿初期血源性感染相关因素。结果影响血源性感染的多因素包括低蛋白血症、PICC导管定期维护、中性粒细胞与淋巴细胞数量(P<0.05)。结论化疗初期应根据患儿血常规和肿瘤细胞水平选择使用,同时为提高患儿免疫功能和营养水平,制订具有促进消化和营养吸收的饮食方案,并定期巡访通知患儿家长进行PICC置管维护。Objective To explore the relevant factors of haematogenous infections in the early stage of neuroblastoma chemotherapy in children,and to remind nursing staff to be highly vigilant to avoid the occurrence of this adverse event.Methods We selected 150 cases of children patients with neuroblastoma chemotherapy who were admitted to the department of pediatric oncology at a tertiary hospital in Guangzhou from August 2016 to August 2019.Among them,27 cases of haematogenous infections occurred and the blood routine,combination of chemotherapy and related data of adverse reactions of all children were collected.Univariate and multivariate logistic regression analysis was used to analyze the related factors that lead to early haematogenous infection in children patients with neuroblastoma chemotherapy.Results The multiple factors affecting haematogenous infections included hypoproteinemia,regular maintenance of PICC catheters,and the number of neutrophils and lymphocytes(P<0.05).Conclusion In early stage of chemotherapy,it should be selected according to the level of children'blood routine and tumor cell.At the same time,in order to improve the immune function and nutritional level of the children patients,a diet plan with promotion of digestion and nutrient absorption should be formulated,and the regular visit should be performed to notify the parents of the children to carry out the maintenance of PICC tube.
