空军飞行人员白内障三焦点人工晶体植入术后放飞2例及航空医学鉴定

摘　　要：目的探讨空军飞行人员白内障摘除联合三焦点人工晶体植入术后的飞行适应性及航空医学的鉴定原则。方法分析2例空军飞行人员单眼白内障的临床诊断、治疗过程及医学鉴定情况,并复习相关文献。结果 1例运输机飞行员及1例空中战勤人员诊断为单眼白内障,术前最佳矫正远视力分别为0.1和0.2。具备手术指征后行白内障超声乳化摘除联合三焦点人工晶体植入术。术后3个月复查,飞行员远视力0.9,中视力0.4,近视力0.8、对比敏感度在正常范围;战勤人员远视力1.0,中视力0.8,近视力0.5,对比敏感度在正常范围;均符合军事飞行人员体检标准,给予飞行合格结论。术后6个月随访正常参加飞行,未见不良反应及飞行不适。结论空军飞行人员白内障行三焦点人工晶体植入术后远中近视力改善明显,是解决空军飞行人员白内障的有效方法。可于手术后3个月进行飞行鉴定,合格者可考虑放飞。Objective To explore the adaptability of Air Force aircrews to flight after trifocal intraocular lens transplantation and related principles of aeromedical evalution. Methods Data on clinic diagnosis, treatment, recovery and aeromedical evaluation of two Air Force aircrews with cataract was analyzed while related literature was reviewed. Results One pilot and one aircrew had been diagnosed with cataract whose visual cauity was 0.1 and 0.2 respectively. No contraindications were found during the ophthalmic examination before trifocal intraocular lens implantation.Three months after surgery, the pilot had normal vision whose distance visual acuity was 0.9, intermediate visual acuity was 0.4, near visual acuity was 0.8 and contrast sensitivity was also in the normal range, compared with 1.0, 0.8 and 0.5 for the aircrew. Flight qualification was given according to the pilots’ standard of health inspection. No adverse reaction or discomfort occurred during the follow-up. Conclusion Trifocal intraocular lens implantation is an effective method to improve the visual acuity of Air Force aircrews with cataract. Flight qualification may be considered three months after surgery.
